糖尿病患者血浆载脂蛋白定量变化与糖尿病肾病相关性研究

摘　　要：目的研究血浆载脂蛋白定量变化与糖尿病肾病的相关性。为糖尿病肾病的早期预防提供可靠的监测方法。方法选择无糖尿病肾病的糖尿患者300例,排除影响肾病的其他因素,一年后按尿蛋白排出率分为无肾病组及肾病组,比较分析两组间血脂指标及血浆载脂蛋白A1、B、A1/B的变化。结果糖尿病肾病组ApoA1减低,ApoB增高,在未发生肾病时即有变化,差异有统计学意义,发展为肾病后,以上变化更为明显。结论糖尿患者血浆载脂蛋白定量变化与糖尿病肾病有相关性,ApoB>1g/L,ApoA1/ApoB<1,可预测、评估糖尿病肾病发生风险,极早干预,此指标可做为降脂治疗的指导指标,以延缓肾病的进展。Objective To study the relevance of quantitative change of plasma apolipoprotein and diabetic nephropathy.Provide reliable monitoring method for diabetic nephropathy early prevention.Methods choose no diabetic nephropathy diabetes 300 cases,eliminate the influence of other factors of kidney,one year later,3divide into the nephrotic group and without nephrotic group according to urinary protein exclude rate,analysis changes between the two groups lipid index and plasma apolipoprotein A1,B,A1 /B.Results ApoA1 decrease and ApoB increase in diabetic nephropathy group before nephrosis.The data show statistical significance.The changes were more apparent when nephrosis happens.Conclusion There is relativity between diabetes human plasma apolipoprotein quantitative change and diabetic nephropathy,ApoB〉1 g /L,ApoA1 /ApoB〈1,the risk of diabetic nephropathy can be predicted and evaluated,extremely early intervention.the index can be used as cholesterol-lowering therapy guidance index to delay the progress of nephrosis.
