重组生长激素对梗阻性黄疸大鼠肝细胞凋亡的影响

摘　　要：目的观察梗阻性黄疸状态下大鼠肝脏细胞凋亡的情况,探讨重组生长激素注射液的抗凋亡作用及其对肝脏的保护作用。方法健康雄性SD大鼠72只,随机分为3组:假手术对照组(SO组,n=24)、梗阻性黄疸模型组(OJ组,n=24)、生长激素治疗组(RH组,n=24)。分别于建模后3、7、14 d随机从各组中取一亚组处死大鼠进行指标测定,检测血清总胆红素(TBIL)和总胆汁酸(TBA)水平变化及肝脏组织学的光镜检查,TUNEL法检测肝组织细胞原位凋亡,免疫组织化学(SP)法检测凋亡相关基因Bcl-2蛋白表达。结果 RH组和OJ组血清TBIL、TBA水平,术后3 d时即明显升高,此后升高渐趋缓慢,并逐渐下降,但仍高于SO组。RH组术后3、7 d时TBIL和TBA水平较OJ组明显下降(P<0.05);组织学光镜观察:RH组和OJ组术后3 d即出现肝细胞损伤,并随时间延长而加重,但各时相点RH组较OJ组减轻。OJ组和RH组术后3 d即见细胞凋亡增加,7d明显增加,14 d后镜下视野见广泛细胞凋亡,RH组与OJ组比较,各时相点凋亡细胞明显减少(P<0.05)。Bcl-2蛋白表达的规律与凋亡呈一致性,但各时相点上表达RH组较OJ组明显上调(P<0.05)。结论大鼠梗阻性黄疸时肝脏存在着细胞凋亡,是梗黄状态下肝功能损害的重要机制。重组生长激素对肝细胞凋亡有明显抑制作用。【Objective】 To observe the hepatic apoptosis in rats with obstructive jaundice（OJ）,and the beneficial effects of Somatropin.【Methods】 Seventy two male SD rats were randomly divided into 3 groups： the sham operated group（SO,n =24）,the obstructive jaundice group（OJ,n =24） and the Somatropin treated group（RH,n =24）.After bile duct ligation,the group RH were treated by Somatropin injections（0.8 IU/Kg/d）,while the group SO and OJ were treated by physiological saline.The blood and hepatic tissues were respectively obtained on the 3 rd,7rdand 14 rd day after operation.The serumal total bilirubin（TBIL） and total bile acid（TBA） were measured by routine methods,the hepatic apoptosis were investigated by using TUNEL,and the expression of Bcl-2 were determined by using immunohistochemical SP methods.【Results】 The serumal TBIL and TBA obviously increased on the 3rd day after operation in both group OJ and group RH,and then decreased after this time,The serumal TBIL and TBA in group RH were obviously lower than group OJ（P 0.05） at every time point.The longer the time of OJ in the rats,the higher the level of hepatic apoptosis in group OJ and group RH,and the apoptosis index（AI） reached climax on the 14 th·day.Compared with group OJ,less hepatic apoptosis occurred in group RH at every time point（P 0.05）.The expression of Bcl-2 was consistent with hepatic apoptosis,but compared with group OJ,the expression of Bcl-2 was obviously up-regulated in group RH at each time point（P 0.05）.【Conclusions】 Hepatic apoptosis plays an important role in the hepatic injury of rats with obstructive jaundice.Somatropin exerts beneficial effects on hepatic cells by anti-apoptosis mechanism.
