摘　　要：目的:评价我院静脉药物配置中心审核抗肿瘤药调配医嘱中发现的不合理医嘱及其干预结果,为提高医院安全用药水平提供参考。方法:提取我院中心配液室2010年1-8月审核发现的抗肿瘤药调配不合理医嘱648组,进行归纳、汇总和分析。结果:不合理医嘱的主要问题为溶媒用量不合理(占30.71%)、溶媒选择不合理(占10.19%),其他有提交错误、包装规格及途径选择不合理等。结论:药师审核医嘱十分必要;医师还应加强对药品说明书内容的掌握,进一步提高医嘱的准确性,确保患者用药安全。OBJECTIVE：To evaluate improper medication orders of antitumor drugs audited by pharmacists of PIVAS in our hospital,and to provide reference for improvement of safe drug use.METHODS：648 improper medication orders of antitumor drugs collected from our hospital from Jan.to Aug.in 2010 were classified,summerized and analyzed.RESULTS：The main problems of the improper medication orders were improper solvent volume （accounting for 30.71%） and improper solvent selection （accounting for 10.19%）.Besides,there were other problems like processing error,incorrectly choosing dosage forms and the route.CONCLUSION：It is necessary for pharmacists to audit medication orders and doctors should be more familiar with package inserts of drugs so as to enhance the accuracy of medication orders and ensure the medication safety of patients.
