系统性红斑狼疮患者CRP水平的分析及临床意义

摘　　要：目的 探讨系统性红斑狼疮（SLE）患者C-反应蛋白（CRP）水平变化及临床意义.方法 回顾性分析2008年8月～2010年8月苏州大学附属第一医院风湿科住院的205例SLE患者,按影像学有无浆膜腔积液表现将其分为有、无浆膜腔积液组,按SLEDAI评分分为有、无活动两组,按统一法获得各观察参数指标数据,分析CRP水平与SLE活动性及浆膜腔积液的关系,并比较分析ESR、C4、C3、IgG与浆膜腔积液的关系.结果 通过分析205例SLE患者,有积液组的CRP水平明显高于无积液组（P〈0.001）,活动组的CRP水平与非活动组无明显差异（P〉0.05）,ESR、C4、C3、IgG与浆膜腔积液无相关（P〉0.05）,通过Logistic回归分析,CRP水平升高与浆膜腔积液的存在有显著相关性（OR= 86.04,95%CI17.85～373,P〈0.001）.结论 SLE患者的CRP水平在反映疾病活动性方面意义不大,但CRP水平升高与浆膜腔积液的存在显著相关.Objective To investigate the changes and the clinical significance of serum levels of CRP in patients with systematic lupus erythematosus（SLE）.Methods Retrospective analysis was done on the characteristics of serum levels of CRP in 205 SLE patients in the Frist Hospital Affiliated to Soochow University, between 2008-10 and 2010-10,in which,some belong to the teams with serous cavity hydrops in iconography and others no;some were cases of activities on the SLEDAI2000,and others were no.By obtaining levels of every observed index,we analysed the relation between serum levels of CRP and serous cavity hydrops,serum levels of CRP and disease activity, as well as blood C3,C4,IgG,ESR and serous cavity hydrops. Result Through the analysis in the 205 SLE patients,we found that the serum levels of CRP were elevated distinctly in SLE patients with serous cavity hydrops comparing to those with no（P〈0.001）;the blood CRP of SLE patients has no difference between the cases of activities and remissions（P〉0.05）,the levels of ESR, C3,C4,IgG in SLE patients has 11o pertinency with serous cavity hydrops（P〉0.05）.By Logistic regression analysis we found that elevator of blood CRP has a markedly relativity with serous cavity hydrops 0R=86.04,95%CI（17.85 ~ 373）,P〈 0.001.Conclusion The serum levels of CRP in SLE patients has a insignificance of disease activity,however, it has a marked relativity with serous cavity hydrops.
