摘  要:目的评价MRI在胎儿中枢神经系统畸形中的应用价值。资料与方法 22例孕妇,年龄23~40岁,年龄平均28岁;孕龄23~40周,平均30周。产前超声(US)检查后48 h内行MRI检查,T2WI采用半傅里叶单激发快速自旋回波序列(HASTE)、真稳态进动快速成像序列(True FISP),T1WI采用超快速小角度激发序列(FLASH)(Turbo FLASH)扫描,将产前MRI、US表现与产后诊断或尸检结果对照。结果产前MRI与US诊断一致并正确者9例,产前MRI正确并证实US可疑病变2例,产前MRI正确并完善产前US诊断1例,产前MRI正确并改变产前US诊断7例,产前US正确MRI不正确1例,产前US正确且完善MRI诊断2例。结论 MRI对胎儿神经系统畸形的诊断具有一定的优势,与US相比,MRI可提供更全面、更准确的信息,甚至更正US诊断,可作为胎儿中枢神经系统畸形的一种重要检查方法和补充手段。Objective To evaluate the value of MRI in fetal central nervous system abnormalities.Materials and Methods The prenatal MRI and US of 22 fetal central nervous system abnormalities were analyzed retrospectively.Twenty two pregnant women,aged from 23-40 years(average 28 years) and with gestation from 23-40 weeks(average 30 weeks) were studied with MRI within 48 hours after US examination.T2-weighted MRI imaging was performed using HASTE and True FISP,T1-weighted MRI imaging was using Turbo FLASH.Prenatal MRI and US findings were compared with autopsy and postnatal diagnoses.Results MRI and US findings were in complete agreement with postnatal diagnoses in 9 cases.MRI correctly certified the US suspected diagnosis in 2 cases.MRI provided additional information to the US diagnosis in 1 case and corrected the US diagnosis in 7 cases.The MRI diagnosis was incorrect and the US diagnosis was correct in 1 cases.US correctly provided additional information to the MRI diagnosis in 2 cases.Conclusion MRI has advantages to evaluate the fetal central nervous system.Compared with US,MRI can provide more accurate and complete information,even correct US diagnosis.MRI can be an important examination for fetal central nervous system abnormalities.
