128层螺旋CT血管成像在颈脑血管疾病中的诊断价值

摘　　要：目的探讨128层螺旋CT血管成像在颈脑血管疾病中的诊断价值。方法收集2009年3月至2010年9月,我院临床怀疑颈脑血管疾病981例患者,应用128层螺旋CT进行颈脑血管成像检查,回顾性分析颈脑血管疾病的各种影像学征象。结果颅内动脉瘤89例,颅内动静脉畸形25例,颈脑血管狭窄524例(颈总动脉狭窄53处,颈内动脉起始段狭窄183处,烟雾病3例,椎动脉起始段狭窄73处,左侧锁骨下动脉起始段狭窄75处,颅内动脉狭窄195处),颅内乙状窦血栓3例,海绵窦瘘3例,血管变异78例,无异常发现259例,阳性检查率73.59%(722/981)。结论 128层螺旋CT血管成像诊断颈脑血管疾病,具有高效、无创、诊断准确等优点,对评价其风险,制定治疗计划有重要的临床应用价值。Objective To investigate the diagnostic value of 128-slice spiral CT angiography in cervical-cerebral vascular diseases.Methods The data of 981 patients who received 128-slice spiral CT cervical-cerebral angiography in our hospital from March 2009 to September 2010 were analyzed retrospectively.Results Among the 981 cases,89 cases were identified with intracranial aneurysm,25 cases with intracranial arteriovenous malformations,524 cases with cervical-cerebral angiostenosis（there were 195 stenoses in internal carotid,183 stenoses in the initial segment of internal carotid,75 stenoses in the initial segment of the left subclavian artery,73 stenoses in the initial segment of vertebral artery,53 stenoses in common carotid artery,and 3 cases of moyamoya disease）,3 cases had sigmoid sinus thrombus,3 cases had cavernous sinus fistula,and 78 cases had variation.The left 259 cases showed normal manifestation.Positive rate was 73.59%（722/981） in this study.Conclusion 128-slice spiral CT angiography has the great advantages in the diagnosis of cervical-cerebral vascular diseases,such as high effectiveness,non-invasiveness,sound diagnostic value.It also can evaluate the risk of diseases and help select appropriate treatment strategies.
