应用纤维膀胱镜联合钬激光治疗膀胱肿瘤的临床研究

摘　　要：目的探讨应用纤维膀胱镜联合钬激光经尿道膀胱肿瘤切除术的临床疗效和安全性。方法对52例膀胱肿瘤(Tis～T2a期)患者应用纤维膀胱镜联合钬激光经尿道膀胱肿瘤切除,其中44例为初发肿瘤,8例为复发肿瘤。肿瘤自瘤体基底部切除,再将其周围1.0～2.0 cm正常组织汽化、烧灼。结果本组52例膀胱肿瘤均1次切除,手术时间10～70 min,平均26 min。术中无闭孔神经反射、膀胱穿孔及水中毒发生,无输血。术后保留导尿时间1～3 d。5例随访3个月,47例随访6个月,均未见肿瘤复发。结论应用纤维膀胱镜联合钬激光经尿道膀胱肿瘤切除,疗效确切、微创、安全性高。Objective To discuss the clinical effect and safety of transurethral holmium laser resection in the treatment of bladder tumors with fiber cystoscope.Methods 52 patients with bladder tumors（stage Tis～T2a） was treated by holmium laser resection with fiber cystoscope transurethrally.Among them 44 patients were primary tumor and 8 patients were recurrent tumor.Tumors were removed from the bases,then the normal tissues which around tumors 1.0～2.0 cm were vaporized and cauterized.Results The tumors were removed once time in all 52 patients.The operation time was during 10 to 70 min（mean,26 min）.No complications such as obturatornerve reflex,bladder perforation,or water intoxication occurred.No blood transfusion was required.The postoperative catheterization time was 1～3 days.No recurrence was found during follow-up examinations for 3 months in 5 patients and 6 months in 47 patients.Conclusion The method that transurethral holmium laser resection with fiber cystoscope might be a safe,effective and minimal invasions procedure in the treatment of bladder tumors.
