治疗高血压性脑出血手术方式的临床探讨

摘　　要：目的探讨治疗高血压性脑出血的手术方式及适应证。方法根据不同病情灵活采用3种手术方式治疗高血压性脑出血。结果本组63例中,大骨瓣开颅血肿清除术14例,治愈13例,死亡1例为术后脑内再出血;小骨窗开颅清除血肿34例,治愈33例,死亡1例,为严重肺部感染;微创锥颅抽吸引流术15例,治愈14例,死亡1例为脑干再出血导致功能衰竭。结论根据患者病情选择不同手术方式治疗高血压性脑出血效果良好,尤其早期手术效果明显。Objective To discuss surgery ways and indications of hypertensive cerebral hemorrhage.Methods There were 3 three kinds of surgical treatment application ccording to different conditions in hypertensive cerebral hemorrhage.Results 63 cases were observed.Among them,the large bone flap hematoma evacuation in 14 cases,13 cases were cured and 1 died of postoperative intracerebral rebleeding.Small bone window craniotomy to remove the hematoma in 34 cases,33 cases were cured and 1 died in serious lung infection.Skull minimally invasive aspiration and drainage in 15 cases,14 cases were cured,1 died as a result of brain stem rebleeding.Conclusion It has good effect in the treatment of hypertensive cerebral hemorrhage to choose different surgical method depending on different patients conditions especially in early surgery.
