系统性硬皮病心肌损害在冷诱导心肌断层显像和超声心动图的表现

摘　　要：应用９９ＭＴｃ－ＭＩＢＩ对２３例系统性硬皮病（ＳＳＣ）病人进行了心肌断层显像冷诱导试验，并同期应用二维超声心动图检测左心室功能和室壁运动。结果表明，１４例病人（６０．９％）有心肌放射性分布异常，其中３例可逆性缺损６个节段，５例固定性缺损９个节段，６例两种缺损共存１４个节段；心肌显像异常率在雷诺现象（ＲＰ）组和无ＲＰ组之间无显著性差异（０．５＞Ｐ＞０．２５）；心肌显像异常组病人的左室射血分数、每搏输出量、心输出量在冷试验即刻较静息时显著降低（Ｐ＜０．０５），且与心肌显像正常组比较也有显著性降低（Ｐ＜０．０５）；超声心动图示４例病人有局部室壁运动低下，部位与心肌图像上可逆性缺损区域一致。提示应用心肌断层显像冷试验估价ＳＳＣ病人的心肌损害有较高的临床价值。The study was performed with cold-induced ~99mTc (MIBI) myocardial imaging(MI) in 23 patients with systematic scleroderma. The left ventricular function andwall motion were also observed by dimensional echocardiography(UCG). 14 patientshad myocardial perfusion abnormalities visualized by MI, including 5 cases with fixeddefects of 9 segments, 3 cases with reversible defects of 6 segments and 6 cases withboth fixed and reversible one of 14 segments. The positive rate in myocardial imaginghad no significant differences between patients with and without Raynaud'sphenomenon (0.5>P>0.25). Compared with baseline, the ejection fraction, strokevolume, cardiac output were significantly decreased during cold--induced in patientswith abnormal myocardial scinti-graphy (P<0.05), and had significant differencecompared with normal group (P<0.05). 4 cases with cold-induced reversibleperfusion defects had anatomically correlated regional ventricular hypokinesis inUCG.
