老年呼吸病房AECOPD病原菌分布和药物敏感性分析

摘　　要：目的了解老年呼吸病房慢性阻塞性肺疾病急性加重期(AECOPD)患者病原菌的分布特点和对常用抗菌药物的敏感性、耐药性情况,为临床合理选择抗菌药物提供参考。方法回顾性分析老年呼吸病房2008年1月至2009年12月218例AECOPD患者病原菌的分布和药物敏感性试验结果。结果 218例AECOPD患者共培养出致病菌171株。革兰阴性菌144株,占84.21%,其中肠杆菌最多(56株),其次为假单胞菌(41株)和不动杆菌(26株);革兰阴性菌对亚胺培南和阿米卡星敏感率较高,对多种常用抗菌药物敏感率均较低。革兰阳性菌24株,占14.04%,其中葡萄球菌最多。真菌3株,占1.75%。结论革兰阴性杆菌是老年呼吸病房AECOPD患者的主要致病菌,对亚胺培南的敏感率较高,对多种常用抗菌药物的敏感率低,耐药率高。应加强细菌耐药性监测,合理选用抗菌药物,提高临床疗效,减少耐药菌的产生。Objective To find out the pathogen distribution,sensitivity and resistance to common antibiotics about patients with acute exacerbation of chronic obstructive pulmonary disease（AECOPD） in the elderly respiratory ward,so as to offering reference to appropriate antibiotic selection.Methods Analysing pathogen distribution and drug resistence of 218 AECOPD patients in elderly respiratory ward of our hospital from Jan.2008 to Dec.2009 by retrospective analysis.Results In 218 cases,we acquired 171 pathogens.In these pathogens,144 strains were gram-negative,the proportion was 84.21%,the most common one in this group was enterobacter（56 strains）,the second was pseudomonas（41 strains） and the third was acinetobacter（26 strains）.Gram-negative bacteriums were highly sensitive to imipenem and amikacin,insensitive to several kinds of common antibiotics.Twenty-four strains were gram-positive,the proportion was 14.04% and the most common one was staphylococcus.Three strains were fungi,which occupied only 1.75% of the whole strains.Conclusion Gram-negative bacteriums were the main pathogens for AECOPD patients in our elderly respiratory ward,which were highly sensitive to imipenem,but insensitive to most of other common antibiotics.So we have to monitor the condition of bacterial drug resistance,choose the suitable antibiotics,enhance therapeutic effect and reduce the generation of drug resistence.
