椎弓根钉棒系统治疗Denis II型骶骨纵形骨折的临床研究

摘　　要：目的探讨应用椎弓根钉棒系统治疗DenisⅡ型骶骨纵形骨折的临床疗效。方法回顾性分析自2006年12月至2009年6月收治的12例DenisⅡ型骶骨纵形骨折患者资料，男8例，女4例；年龄23～55岁，平均35岁。应用椎弓根钉棒系统进行复位及固定，根据术后随访的临床体征和症状以及x线检查结果评估其安全性和疗效。结果12例患者术后获8～24个月（平均15个月）随访。手术时间1．5～2．5h，平均2．0h；输血200～500mL，平均350mL；术后住院时间为10～18d，平均15d。10例患者术后双下肢等长，2例出现患肢缩短（2cm），行走有轻度跛行。术后x线片示钉棒系统位置良好。术后所有患者骨折端均获一期骨性愈合。Majeed评分：满意10例，可2例。结论应用椎弓根钉棒系统治疗DenisⅡ型骶骨纵形骨折，疗效良好，是一种安全、有效的手术方法。Objective To explore clinical effects of pedicle screw-rod instrumentation in treatment of Denis II vertical sacral fractures. Methods From December 2006 to June 2009, we used pedicle screw-rod system to treat 12 cases of Denis II vertical sacral fractures. They were 8 men and 4 women, aged from 23 to 55 years （mean, 35 years）. The therapeutic effects and safety of pedicle screw-rod instrumentation were assessed according to the results of their follow-ups and X-ray examinations. Results The 12 cases were followed for an average of 15 months （range, 8 to 24 months）. The symptoms improved obviously for all the patients and their fractures obtained bony fusion. X-ray examinations revealed fine fracture reduction and internal fixation in 10 cases. A shortening of 2 em in the affected limb was observed in 2 cases after fracture union. Conclusion Pediele screw-rod instrumentation is a safe and effective surgical treatment for Denis II vertical sacral fractures.
