胰岛素抵抗分布特点与心血管代谢危险因素的关系

摘  要:[目的]研究胰岛素抵抗(IR)与心血管代谢危险因素的相关性.[方法]于2008年8月至2009年8月在延边朝鲜族自治州安图县内采用简单随机抽取方法抽取1 235名常住居民,对其进行健康体格检查和生物化学指标检测.用放射免疫法检测空腹胰岛素(FINS),采用稳态模式评估法估测人体IR水平.血脂及血糖指标均用全自动生物化学分析仪进行测定.[结果]有IR、高血糖、高血压、血脂异常、整体型肥胖及腹型肥胖者的比例分别为25.0%,14.4%,37.5%,23.8%,34.5%,61.3%.简单相关分析及调整民族、性别及年龄因素的影响后偏相关分析结果均显示,SBP,DBP,TC,TG,BMI,WHtR,FBG,FINS均与IR相关(P<0.01).具有2种以上代谢异常者比例随着IR水平增高而上升,且具有2种、3种及4种代谢异常者比例也随着IR水平增高而上升(P<0.01).[结论]延边地区一般人群中IR和常见心血管代谢危险因素的检出率均较高,而且IR者伴发常见代谢异常的现状普遍存在;IR水平与不同代谢异常聚集者的比例呈正相关.OBJECTIVE To study the relationship between insulin resistance(IR) and risk factors of cardiovascular metabolism.METHODSA total of 1235 adult subjects were simple randomly selected from the Antu county,Yanbian Korean Autonomous Prefecture,and the anthropometric and biochemical indexes were measured.The fasting insulin(FINS) was detected by radioimmunoassay,and human IR was estimated by the homeostasis model assessment method,and blood lipids and glucose were measured by automatic biochemical analyzer.RESULTSThe proportion of IR,hyperglycemia,hypertension,dyslipidemia,overall obesity and abdominal obesity was 25.0%,14.4%,37.5%,23.8%,34.5% and 61.3%,respectively.After simple correlation analysis and adjusting influencing factors for ethnicity,gender and age,the SBP,DBP,TC,TG,BMI,WHtR,FBG and FINS were all correlated with IR(P0.01).The proportion of patients suffered from two or over two kinds of abnormal metabolism increased along with the increase of IR level(P0.01).CONCLUSIONThe prevalence of IR and risk factors for cardiovascular metabolism in the general population is relatively high in Yanbian area,and the insulin resisters with common dysbolism are ubiquity.The frequency of IR increases significantly along with increase of dysbolism composition.
