摘  要:目的探讨巨细胞病毒(CMV)感染新生儿尿DNA含量和血T细胞亚群的变化及意义。方法选择CMV症状性感染新生儿20例,CMV无症状性感染新生儿23例,健康查体新生儿16例,检测各组尿DNA含量和血T细胞亚群的水平,并对CMV感染病儿进行随访。结果 CMV症状性感染组尿DNA含量明显高于CMV无症状性感染组,差异有显著性(t=3.698,P〈0.01)。CMV症状性感染组CD3+、CD4+的水平和CD4+/CD8+的比值较无症状性感染组和健康对照组均降低,CD8+水平较其他组升高,差异有显著性(F=3.74~42.34,q=3.21~11.76,P〈0.05、0.01)。随访显示CMV症状性感染组病儿中6例T细胞亚群的检测结果持续异常,并且肝功能反复,其中4例病人尿CMV DNA含量升高并维持在较高水平。结论 CMV症状性感染组尿DNA含量和血T细胞亚群较无症状性感染组有明显变化,此在判断预后和指导临床治疗方面有一定意义。Objective To investigate the changes and significance of DNA content in urine and T cell subsets in blood of neonates infected with cytomegalovirus(CMV).Methods Forty-three neonates who were confirmed infected by CMV were selected in this study: 20 cases were symptomatic,and 23 asymptomatic.Sixteen newborns who received health examination served as controls.The DNA content in urine and T cell subsets in blood were detected.The sick neonates were followed up.Results The urine DNA level in symptomatic sick neonates was higher than that in the asymptomatic,the difference being significant(t=3.698,P0.01).Blood levels of CD3+,CD4+ and CD4+/CD8+ in the symptomatic group were lower than that in the asymptomatic and healthy controls,while the levels of CD8+ were higher(F=3.74-42.34;q=3.21-11.76;P0.05,0.01).Follow-up revealed that of the symptomatic sick neonates,six of them showed persistent T cell subsets,accompanying abnormal liver functions,of whom,urine DNA elevated in four cases and which was kept at a higher level.Conclusion Marked changes of urine DNA and blood T cell subsets were observed in symptomatic CMV-infected neonates,which is of significance in prediction of prognosis and in guidance of clinical therapy for this condition.
