胃管自然引流在肝移植术后应用的效果观察

摘　　要：[目的]探讨肝移植术后胃管拔除时间。[方法]随机选取我院2006年—2009年肝移植手术病人80例,分成两组,观察组术后胃管自然引流,不施行负压,待胃液<200 mL/d,可听及肠鸣音后拔除胃管。对照组给予持续胃肠减压,于肛门排气后拔除胃管。比较两组病人术后进食时间、排气时间、恶心呕吐、腹胀腹痛、病人舒适程度等指标。[结果]两组病人在手术后拔除胃管时间、病人舒适程度、进食时间、睡眠等指标观察组显著优于对照组。[结论]肝移植病人手术后胃管自然引流,待胃液<200 mL/d,可听及肠鸣音后拔除胃管可增加病人舒适度,降低相关并发症,并早期恢复进食,有利于病人的术后康复。Objective：to probe into gastric canal extracted time of patients after undergoing liver transplantation.Methods： A total of 80 patients undergoing liver transplantation from 2006 to 2009 were randomly selected and divided into experimental group and control group,experimental group patients were given natural drainage by gastric canal after operation,no negative pressure was exerted,when the gastric juice was less than 200 ml/d and the bowel sound was heard,the gastric canal was extracted.The control group patients were given persistent gastrointestinal decompression,and the gastric canal was extracted after the passage of gas by anus.Then the postoperative eating time and passage of gas time,nausea and vomiting,abdominal distension and stomachache,easement level of patients were compared.Results： there was no statistical significant difference in postoperative abdominal gas pains,nausea and passage of gas time by anus between both groups,however,the gastric canal extracted time,easement level of patients,eating time and sleep of experimental group were much better than control group.Conclusion：natural drainage by gastric canal after operation and when the gastric juice was less than 200 mL/d and the bowel sound was heard,the gastric canal was extracted which can increase the easement level of patients,reduce related complications recover the eating earlier,and is profit for patients＇postoperative rehabilitation
