主动脉瘤囊-右心房分流术在主动脉手术中的应用

摘　　要：目的探讨主动脉瘤囊-右心房分流术在主动脉手术中的应用效果。方法回顾性分析我院2005年7月—2009年8月45例主动脉手术,其中应用主动脉瘤囊-右心房分流术18例。总结手术时间、主动脉阻断时间、术后引流量、术后输血量、二次开胸发生率。结果与未分流术组病人相比,除主动脉阻断时间及二次开胸率外,分流术组病人的手术时间短,术后第1天及术后总引流量少,术后输注红细胞及血浆量少,差异均有显著性（t=1.96~3.74,P〈0.05）。结论主动脉瘤囊-右心房分流术可以有效减少主动脉手术后出血,缩短手术时间,降低输血量,二次开胸率较低,且操作简单,值得临床推广应用。Objective To discuss the effect of aortic aneurysmal sac-right atrial shunt in the operation of aorta. Methods The records of 45 aortic operations done in our hospital,from 07-2005 to 08-2009,were analyzed retrospectively,of which,the aortic aneurysmal sac-right atrial shunt procedure was applied in 18 cases.The operation time,blockage time of the aorta,postoperative drainage volume,postoperative blood transfusion and re-thoracotomy rate were summarized. Results Compared with those underwent surgery not using the shunt technique,the shunt technique showed the advantages of shorter operation time,less drainage volume,and less postoperative blood transfusion,the differences between the two groups being significant,except the time of aorta blocking and re-thoracotomy（t=1.96-3.74,P0.05）. Conclusion The aortic aneurysmal sac-right atrial shunt can effectively reduce blood loss after surgery,shorten the operation time,and decrease blood transfusion;the incidence of re-thoracotomy is low.The shunt procedure is simple to perform and worth clinical generalization.
