解剖型锁定钢板治疗股骨粗隆间骨折的效果

摘　　要：目的探讨股骨上段解剖型锁定钢板锁定股骨颈内侧皮质在治疗各型股骨粗隆间骨折中的效果。方法 2008年6月—2010年3月,我科对13例股骨粗隆间骨折病人,采用闭合复位解剖型锁定钢板内固定术治疗,骨折近端采用3枚自攻长锁定螺钉固定,其中1枚拧入股骨颈内,余2枚经股骨颈并穿出内侧皮质;远端采用自攻锁定螺钉和普通螺钉固定。结果 13例病人获得3～12月随访,骨折均愈合,无明显髋内翻畸形及内固定物松动、断裂等;根据股骨粗隆间骨折的SANDERS创伤性髋关节评定标准:优12例,良1例,优良率100%。结论解剖型锁定钢板锁定股骨颈内侧皮质治疗股骨粗隆间骨折,创伤小,手术时间短,固定可靠,并发症少,骨折愈合率高,病人能早期活动,临床效果优良。Objective To explore the effect of anatomical locking plate in the treatment of various types of intertrochanteric fractures in femoral neck. Methods In June 2008-March 2010,13 patients with intertrochanteric fractures were treated at our department by closed reduction and internal fixation using anatomic locking plate.The procedures included three self-tapping locking screws were fixed in the proximal end of fracture,one in the femoral neck,the other two into femoral neck and passed the medial cortex;self-tapping locking screws and ordinary screws were used to fix the distal end. Results Postoperatively,all the patients were followed for 3-12 months,which showed that all the fractures were completely healed,with no obvious coxa vara deformity and no loosening or broken of internal fixation.According to SANDERS＇s hip injury assessment criteria： 12 cases were classified as excellent and one as good,the fineness rate being 100%. Conclusion Treatment of intertrochanteric fractures with anatomical locking plate is ideal with mini-trauma,shorter operation time,reliable fixation,less complication,higher union rate of fracture,earlier ambulant and excellent clinical results.
