螺旋CT多平面重建在诊断寰枢椎损伤中的应用

摘  要:目的:探讨螺旋CT多平面重建(MPR)在寰枢椎外伤诊断中的作用,提高对寰枢椎损伤的诊断率。方法:对30例寰枢椎损伤病人进行螺旋CT薄层扫描和MPR、MIP、SSD重建进行比较,分析并评价其对寰枢椎结构的显示效果。结果:单纯寰枢椎脱位9例;枢椎齿状突骨折12例,枢椎骨折4例,其中骨折伴寰枢椎脱位8例,寰椎前、后弓骨折共5例。结论:MPR可准确测量碎骨片大小、移位、方向和程度,但在立体感和整体感方面较SSD、MIP差,笔者认为三维重建中,MPR应用范围最广,显示效果也最好,着重推荐采用MPR和SSD、MIP两种技术相结合的方法,对寰枢椎损伤可达到最满意的诊断目的。Objective: To evaluate spiral CT multi-planar reconstruction (MPR) in diagnosing atlanto-axial injuries to improve the diagnostic rate. Method: 30 cases of atlanto-axial injury with thin spiral CT scan comparing with MPR, MIP, SSD reconstruction. Analysing and evaluatiing its effects in atlanto-axial structure . Result. 9 cases of atlanto-axial dislocation ; 12 cases of odontoid fracture ; 4 cases of atlanto-axial fractures, Fracture with atlanto-axial dislocation in 8 cases , Atlas before and after a total of five cases of arch fracture. Conclusion: MPR can accurately measure the size of fragments of bone , displacement, direction and degree, However, in third-dimension and the wholeness sense less than SSD, MIP. We believe that in the third-dimensional reconstruction , the applications of MPR is the widest , and its results is the best. Focus on recommended method that combining MPR and SSD, MIP. which can achieve the most satisfactory diagnostic purposes in atlanta-axial injuries.
