多层螺旋CT多平面重组诊断听小骨脱位的价值

摘　　要：目的探讨多层螺旋CT多平面重组诊断听小骨脱位的临床价值。方法回顾性分析14例共16耳听小骨脱位的高分辨率CT扫描表现,利用多平面重组技术进行多角度观察,全面评价脱位情况。结果 14例听小骨脱位病例中,单纯锤砧关节脱位1例1耳,表现为锤砧关节间隙增宽;单纯砧镫关节脱位4例4耳,表现为砧镫关节间隙增宽及砧骨长脚移位;锤砧关节和砧镫关节均脱位9例11耳,除表现为锤砧关节和砧镫关节间隙增宽外,还可见不同程度的砧骨移位。结论多层螺旋CT多平面重组可以很好地显示听小骨脱位情况,为临床制定治疗方案提供重要信息。Objective To assess the value of multi-slice CT multiplanar reconstruction in the diagnosis of ossicular dislocation（OD）.Methods A retrospective was done of CT manifestations of 16 OD in 14 patients using multiplanar reconstruction technique to observe and overall assess the dislocations.Results Of the 14 OD,simple separation of incudomalleal joint was seen in one,displaying as widening of incudomalleal joint;simple incudostapedial joint dislocation in four;displaying widening of incudostapedial joint and bit shifting of the long-process of incus;dislocation of both the incudomalleal and incudostapedial nine cases（11 ears）,various extent of incus shifting could be seen in addition to widening of incudomalleal and incudostapedial joints.ConclusionMulti-slice CT multiplanar reconstruction can commendably demonstrate ossicular dislocation and provide important information for making therapeutic regimen.
