摘 要:目的探讨速度向量成像(velocity vector imaging,VVI)技术分析双腔起搏(DDD)前后左室心肌运动速度和位移变化规律,初步探讨VVI技术的应用价值。方法 23例DDD起搏患者手术前后均采用VVI技术检测心室各节段心肌纵向运动收缩峰值速度和位移,并比较手术前后差异。结果 23例患者手术前后左室心肌纵向收缩峰值速度与位移均从基底段到心尖段依次递减,不同室壁间以后壁速度及位移最高。与术前相比,术后下壁速度显著降低(P<0.01),心尖段的整体速度显著降低(P<0.01);术后基底段与中间段整体位移显著低术术前(P<0.01),术后左室整体以及侧壁、后间隔与下壁位移显著降低(P<0.05)。结论 DDD起搏前后左室心肌运动速度和位移明显不同,术后较术前明显降低;VVI技术能准确地定量评价DDD起搏前后左室心肌运动。Objective To assess longitudinal segmental myocardial velocity and displacement of left ventricle before and after DDD Pacing using velocity vector imaging(VVI) and to explore the value of clinical application of VVI.Methods The systolic peak velocity and displacement of ventricular segments were measured and compared before and after DDD pacemaker implantation using VVI in 23 patients.Results Left ventricular longitudinal systolic peak velocity and displacement significantly decreased from basal,mid to apical segments before and after DDD pacing(P0.01).The velocity and displacement of posterior wall were higher than those of other segments(P0.01).After DDD pacing,the velocity of inferior and apical segment wall and the displacement of basal and mid segments significantly decreased(P0.01).Moreover,the postoperative evaluation showed that the displacement of left ventricular and lateral wall,interventricular septum and posterior wall were significantly lower than preoperatively(P0.05).Conclusion Left ventricular velocity and displacement significantly decreased after DDD pacing.VVI can accurately assess left ventricular myocardial motion in patients with DDD pacemakers.
