摘  要:目的探讨丙泊酚在射频消融治疗子宫肌瘤中最佳的静脉输注速度及其对呼吸循环的影响。方法 90例子宫肌瘤患者随机均分为A、B、C 3组,按2 mg/kg剂量持续推注丙泊酚作为预冲剂量,术中分别选择给予丙泊酚静脉输注,速度为A组7.5 mg/(kg.h)、B组8.0 mg/(kg.h)、C组8.5 mg/(kg.h)。用多功能生理监测仪分别记录麻醉前、手术中的血压、心率、呼吸、血氧饱和度(SpO2)及脑电双频指数(BIS)。结果 B组使用8.0 mg/(kg.h)的速度持续静脉输注丙泊酚取得了良好的麻醉效果,术中血压、心率、呼吸、SpO2、BIS的变化均在正常范围内,同A组相比没有出现麻醉效果为Ⅲ级的患者,且术中生命体征相对平稳,同C组相比,麻醉药物对心血管系统及呼吸系统的影响更小。结论在射频消融治疗子宫肌瘤手术中行丙泊酚静脉麻醉采用8.0 mg/(kg.h)的速度进行持续输注,可达到良好的麻醉效果。Objective To discuss the optimal speed of Propofol infusion by phleboclysis in hysteromyoma operation performed with radio frequency current ablation,and its effect on respiratory circulation.Methods 90 patients with hysteromyoma were randomly divided into group A,B and C,and each patient received continuing infusion of Propofol at the preliminary dose of 2.0 mg/kg.In the process of operation,the speed of Propofol infusion by phleboclysis for Group A,B and C were 7.5 mg/(kg·h),8.0 mg/(kg·h) and 8.5 mg/(kg·h) respectively.Before anesthesia and during operation,the parameters of BP,HR,RR,SpO2,BIS were recorded by multifunctional physiological monitor.Results With continuing infusion of Propofol at the speed of 8.0 mg/(kg·h),Group B obtained the best anesthesia result.In the whole process of operation,all the parameters of BP,HR,RR,SpO2,and BIS were within the normal range.When compared with group A,no patient was evaluated as grade Ⅲ of anesthesia effect in group B,and their vital signs were comparatively steady.When compared with that of group C,the anesthetics used in group B had less effect on cardiovascular and respiratory systems.Conclusion Continuing infusion of Propofol by phleboclysis at the speed of 8.0 mg/(kg·h) can achieve satisfactory anesthesia effect.
