摘　　要：日本学者Tei于1995年提出了一种无创综合评价心脏整体收缩和舒张功能的超声检查指标-Tei指数（亦称心肌工作指数）,此参数不受心脏几何形态及心脏瓣膜返流等因素的影响[1]。本文分析了Tei指数与心肺运动耐量指标（6分钟步行距离及运动峰耗氧量等指数）的相关性,同时进一步探讨了Tei指数对慢性心力衰竭患者心肺运动耐量的预测价值。Objective To explore the correlation between Tei index and cardiopulmonary exercise capacity in patients with chronic heart failure.Methods 127 patients with chronic heart failure were consecutively recruited during 2008-2010 in cardiovascular clinic of Zhejiang Hospital.All participants underwent 6-minute walk test to determine the 6 minute walk distance.During each test,maximal oxygen consumption（VO2） was measured using a portable metabolic system.Tei index was assessed by Doppler echocardiography.Results A total of 127 patients with chronic heart failure included 33 patients with NYHA I classification,73 patients with II classification,and 21 patients with NYHA III classification.Unadjusted correlation analysis showed that Tei index was negatively related to the 6 minute walk distance（r=-0.19,P0.05） and VO2（r=-0.23,P0.01） respectively.Multiple stepwise regression analysis showed that Tei index（r2=0.08,P0.01） and the early LV filling velocity（r2=0.04,P0.05） were independent determinants of the 6 minute walk distance.Conclusions Tei index was significantly associated with cardiopulmonary exercise capacity in patients with chronic heart failure,and was also an independent determinant for exercise capacity.
