ENA多肽抗体谱检测结合抗ds-DNA及补体C3测定在SLE诊断上的应用

摘　　要：目的探讨抗ENA(可提取抗原)抗体、抗ds-DNA抗体(抗双链DNA)及补体C3测定在系统性红斑狼疮(SLE)的诊断上的临床应用。方法对本院196例确诊为自身免疫性疾病患者的实验室及临床资料进行回顾性分析,其中系统性红斑狼疮(SLE)76例,干燥综合征(SS)14例,进行性系统硬化症(PSS)6例,皮肌炎(DM)8例,风湿性关节炎(RA)92例。抗ds-DNA采用免疫斑点法,抗ENA抗体采用免疫印记法,补体C3测定采用免疫比浊法测定。结果抗ds-DNA、抗Sm、抗r-RNP在SLE中检出率分别为73.6%、27.6%、13.1%.在其他风湿性疾病中未检出,特异性相对较高.抗SSA、抗U1RNP在SLE中的检出率分别为36.8%、27.6%,在其他风湿性疾病亦有检出,特异性相对较低.其他风湿性疾病中SSA、抗SSB、抗U1RNP在SS的检出率分别是:50%、43%、25%;抗SSA、抗Jo-1在PPS的检出率分别为25%、25%;抗SSA、抗U1RNP、抗Jo-1、抗DM53在DM的检出率分别为25%、25%、12.5%、37.5%;抗SSA、抗U1RNP、抗RA/54在RA的检出率分别为32.6%、11.9%、15.2%。补体C3在SLE的测定值均较低,范围为0.46±0.12与其他风湿性疾病相比,P<0.01有显著差异。结论抗ENA抗体结合抗ds-DNA抗体及补体C3测定对SLE的诊断有显著的临床意义。Objective To investigate the anti ENA antibodies,anti ds DNA antibodies and complement C3 detection in the diagnosis of SI.E clinical application. Methods 196 hospital patients with autoimmune diseases diagnosed in the laboratory and clinical data were reviewed, including systemic lupus erythematosus （SIZE） 76 cases,sjogren syndrome （SS） 14 cases, progressive system sclerosis （PSS） 6 cascs,dermatomyositis （I）M） 8 cases,rheumatoid arthritis （RA） 92 cases, anti ds DNA by immuno-spot assay, anti ENA antibodies by Weslern blotting （IBT） ,complement C3 was determined by immuno turbidimetrie method. Results The re suits of anti ds DNA,anti-sm,anti rRNP detected in SLE were 73.6%,27.6%, 13. 1%,not detected in other rheumatic diseases, the specificity is relatively high. Anti-SSA,anti U1RNP in SLE was 36.8 %,27.6%,and other rheumatic diseases are also detected, specificity is relatively low. In other rheumatic diseases the positive rate about anti SSA,anti SS13,anti-U1RNP were 50%,43%, 25% respectively in SS;Thc positive rate about anti-SSA.anti Jo 1 were 25%,25% reslgectively in PPS;The positive rates about anti-SSA,anti UlRNP,anti Jo 1,anti-DM53 were 25%,25%,12.5%,37.5% respectively in DM;the positive rates about anti SSA,anti-U1RNP,anti A/54 were 32.6%,11.9%,15.2% respectively in RF. Complement C3 in the determination of SLE values was low,ranging from 0.46 ± 0. 12,compared with other rheumatic diseases, （P〈0.01） significant differences. Conclusion The combination of anti ENA antibodies, anti, ds DNA antibodies and complement C3 determination in the diagnosis of SLE have signifi cant clinical significance.
