摘 要:目的利用分级液压脑损伤骨折模型研究脑损伤后大鼠血清中神经肽Y(Neuropeptide Y,NPY)的变化及对脑损伤后骨折愈合加速的影响。方法 SD大鼠随机分成三组,空白对照组10只,单纯骨折组48只,脑损伤骨折组144只,脑损伤按轻重分成轻、中、重三组,通过侧方液压脑损伤及开放骨折建立模型,损伤后适时取SD大鼠血清及骨折标本,通过放射免疫荧光、X片及骨折直径及生物特性测量了解血清中NPY的含量、骨折愈合情况、骨痂直径及抗应力的大小。结果单纯骨折组、脑损伤骨折组血清中NPY含量升高,尤以中、重度脑损伤骨折组明显;单纯骨折于第1天达到高峰,为(10.96±4.48)ng/ml,随后下降,而脑损伤骨折组于第3天达到高峰,分别为(22.27±6.48)ng/ml、(34.94±7.12)ng/ml、(56.63±12.36)ng/ml,随后下降,除轻度脑损伤骨折组伤后3h及伤后14天,轻、中、重度脑损伤骨折组血清中NPY含量明显高于单纯骨折组(P<0.05),且时间长;脑损伤骨折组骨痂无论在影像学、直径大小及骨痂抗应力均大于单纯骨折组,术后3、4周X片、骨痂直径及抗应力重度组明显大于中度、轻度及单纯骨折组,而中度组大于轻度组且骨痂致密差异有显著性(P<0.05)。结论本实验结论与临床观察一致,脑损伤骨折大鼠骨折愈合快于单纯骨折,且脑损伤越重血清中NPY含量越高,骨折愈合越快,骨痂量越大,愈合时间短,推测脑损伤轻重可能与NPY含量及骨折愈合速度相关。Objective To study the changes of serum neuropetide Y(NPY) in Sprague-Dawley rats after injury and its effects on accelerating fracture healing by utilizing a graded lateral fluid percussion brain injury associated with bone fracture model.Methods Sprague-Dawley rats were randomly allocated to three groups: the blank control group(n=10),simple fracture group(n=48),brain injury associated with fracture group(n=144).Brain injury associated with fracture group were again randomly allocated to mild,moderate,severe groups by the degree of traumatic brain,establishing the combined Sprague-Dawley rat model of traumatic brain and tibia fracture via fluid lateral percussion and open fracture.After injury,the SD serum rats and bone fracture samples were collected at proper time.The NPY quantity,the bony fracture healing,the diameter of the bone callus and biomechanical testing outcomes c were measured and recorded by using radio-immunity fluorescent technique,X-ray film,the diameter of the callus and biomechanical characterics.Results Serum NPY quantity increased in simple fracture group,brain injury associated with fracture group,especially in moderate and severe brain injury groups.Serum NPY quantity in simple fracture group reached to the peak value at(10.96±4.48)ng/ml on day 1,then decreased. In brain injury associated with fracture group,serum NPY quantities reached to the peak value on day 3,serum NPY quantity in the three subgroups were(22.27±8.48)ng/ml,(34.94±7.12)ng/ml and(56.63±12.36)ng/ml,respectively,then decreased again.Except for 3 hours and 14 days after injury in mild brain injury associated with fracture group,serum NPY quantity in mild,moderate and severe brain injury associated with fracture groups were higher than those in the simple fracture group(P〈0.05),also the time lasted longer.In the brain injury associated with fracture group,no matter in imaging,the diameter of the callus,or the bone callus biomechanical testing records were higher than those in the s
