摘  要:从动物实验对大剂量激素冲击应用与长期应用对于动物机体及股骨头坏死的不同影响进行对比研究,为临床上激素应用后产生股骨头缺血性坏死提供依据,强调股骨头缺血性坏死预防的必要性。采用SD大鼠21只,随机分成3组,剂量参照Wang的实验,第一组臀肌注射醋酸强的松龙49mg/kg·d,每日一次,应用6d;第二组24.5mg/kg,每周一次,应用12周,两组累计剂量均为294mg/kg;第三组为正常对照组。实验期间观察各组动物全身情况,动物处死后观察动物内脏大体情况及内脏、股骨头光镜检查,并对3组切片中的空骨陷窝率进行了统计学处理。结果:①冲击组动物全身情况及内脏损害程度较长期组严重。②长期组的股骨头坏死征较冲击组严重,空骨陷窝率更高,两组的空骨陷窝率与正常组相比均有显著性差异(P<0.01)。结论:①大剂量激素对动物体的毒副作用严重。②大剂量激素应用对全身血液循环系统的损害与股骨头内的血管变化一致。③单纯激素可以产生股骨头坏死。④激素的冲击疗法与长期应用都能促进股骨头坏死的发展,因此必须重视预防。Authors research the different effect to animal body and aseptic necrosis of femoral head (ANFH) after being injected corticosteroids for a long time or a short time with an ultradose, which can provide an experimental basis and warn the necessarity to prevent GANFH. Twenty - one adult SD rats were divided into three groups. Group first consisted of seven animals that received an gultaeus muscle injection of 49 mg/kg.d, prenisoloni Acetatis for six consecutive days. Group second consisted of seven rats that received an glutaeus muscle injection of 24.5 mg/kg prenisoloni Acetatis once a week for twelve consecutive weeks. The total dose of group first and second was 294 mg/kg equally. Group third com-prised seven rats that received isotonic saline. All animals were observed about their reaction, gross and microscopy mani-festation of viscera and femoral head, a statistics about the rate of empty lacunae. Results showed:①The reflection of body and manifestation of gross viscera of group first were worse than those of group second.②The display of ANFH and the rate of empty lacunae of body and manifestation of gross viscera of short-time group were worse than those of long -time group.② The display of ANFH and the rate of empty lacunae in long - time group were worse than that in short -time group, there were significant difference between two groups (P<0. 01). Conclusion: ①The toxicity and by - ef-fect of ultradose corticosteroids were giant.②The damage to vascular system of the whole body for overdose corticos-teroids is accordance with that to blood vessel in femoral head.③GANFH can be brought out by corticosteroids solely.④ The treatment that a ultradose corticosteroids in a short time or a overdose corticosteroids in a long time can promote the development of GANFH. Therefore, we must emphasize the prevention of GANFH.
