前入路经皮穿刺腰5骶1椎间盘髓核摘除术的可行性研究与探讨

摘　　要：目的　研究和探索对腰 5骶 1(L5 /S1)椎间盘突出和突出并髓核迁移病例安全有效的介入治疗新途径 ,拓宽适应证范围。方法　自 1995年 8月至 1998年 12月底 ,通过对 2具尸体、3条成年犬实验 ,以及影像学和相关解剖学资料的对照研究 ,自行设计了压腹定位器 (专利号 98110 0 0 90 )并改良现有后路椎间盘切吸仪器 ,采用消化道造影、下腹局部加压及空心针穿刺技术 ,以逐层套管方式建立手术通道 ,在透视和椎间盘窥镜监控下钳取髓核组织及负压切吸 ,实现椎间盘突出部位直接减压。在动物实验取得成功的基础上 ,共完成L5 /S1椎间盘前路髓核摘除术 78例。结果　 78例患者中 ,总有效率 94 8% ,显效率 85 9%。其中 1例术中造成末段小肠损伤 ,经对症处理而痊愈。本疗法患者体位舒适、痛苦少 ,不造成腰肌和神经根损伤 ,术中可直接摘除突出部位的椎间盘组织。结论　该疗法具有损伤小、安全性高和恢复快等优点 ,为发病率较高的L5 /S1椎间盘突出提供了一种新的介入治疗途径。Objective ToexploreannewsafeandeffectivetreatmentforL5 /S1intervertebraldisc protrusionorextrusionwithshiftingandtoextendthescopeofindicationsforinterventionaltreatment Methods FormAugust 1995toDecember 1998,twocadaversandthreeageddogswerestudiedthroughcomparative experimentsofimagingandrelatedanatomicinformation Aself manufacturedinstrumentpressuringabdomenon segmentallocalization (patentnumber :98110 0 0 90 )wasproducedsoastoimprovethepresentinstrument Introductionofimageofdigestiveorgan ,pressureonthelowerabdomenandpuncturewithhollowneedle ,cannulas onebyonetocreateoperativepassage ,verifythesitebyfluoroscopyanddiscendoscopy ,clipthenucleus pulposus,anddepressderectlythesiteofherniationofdiscwithextractionandsuction Aftersuccessinanimal experiments 78casesweretreatedwiththismethod Results 78caseshaveturnedouttobesuccessful,total efficiencywas 94 8% ,85 8%caseshadexellentoutcome Inonlyonecaseoccurredthedamageofthe interstinumtenuebottomduringthesurgery ,andcuredbyremedyofsymptom Thistreatmentprovidescomfortfor patientsposture,avoidingdamagetothelumbermuscleandnerveroot Theendoftheforcepscanbemanagedto thedisctissureinthesiteofherniationduringsurgerywithlessdamageandpainandquickrecovery Conclusions ThistreatmentprovidesanewapproachtotheinterventionaltreatmentofL5 /S1intervertebraldiscprotrusion
