上消化道出血患者凝血纤溶系统动态变化研究及护理

摘　　要：目的:探讨上消化道出血患者凝血纤溶系统动态变化规律及护理方法。方法:选取2008年7月～2010年4月于本院进行治疗的148例上消化道出血患者为观察组,同时选取同期的148名健康人为对照组,将观察组患者止血前、止血后即刻及止血后24 h和对照组的凝血四项与纤溶三项水平进行统计及比较,并根据患者的实际情况进行相应的护理。结果:经研究比较发现,止血前和止血后即刻观察组的PT、APTT、FIB、TT、D-D均高于对照组,t-PA活性、t-PA抗原、PAI均低于对照组,P<0.05或P<0.01,有显著性差异或有非常显著性差异,而止血后24 h两组人员的PT、APTT、FIB、TT、D-D、t-PA活性、t-PA抗原、PAI比较,均P>0.05,无显著性差异。结论:上消化道出血患者凝血纤溶系统动态变化呈现一定的变化规律,治疗过程中配合较好的护理,可及时止血,应引起重视。Objective：To study the nursing methods and dynamic change law of coagulation system and fibrinolytic system of upper gastrointestinal bleeding.Methods：148 patients with upper gastrointestinal bleeding in our hospital from July 2008 to April 2010 were selected as observation group,and 148 healthy people at the same time were selected as control group,then the coagulation system and fibrinolytic system of the control group and observation group before and after hemostasis immediately and at 24 h were analyzed and compared,and all the patients were well taken care of.Results：The PT,APTT,FIB,TT,D-D of observation group before and after hemostasis immediately were all higher than those of control group,the t-PA activity,t-PA antigen and PAI were all lower than those of control group,P0.05 or P0.01,there were significant differences or there were very significant differences,but the PT,APTT,FIB,TT,D-D,t-PA activity,t-PA antigen and PAI of two groups after hemostasis at 24 h were compared,all P0.05,there were no significant differences.Conclusion：The dynamic change of coagulation system and fibrinolytic system of upper gastrointestinal bleeding shows its law,and better nursing is done during the course of the treatment,and we can stop bleeding in time,and shoul be paid attention to.
