摘 要:目的评价噬菌体生物扩增法(PhaB法)检测结核性液体标本的临床应用价值。方法应用BacT/ALERT 3D培养法(3D培养法)、PhaB法对142例液体标本进行检测,其中结核病例组122例,包括胸腹腔积液69例、脑脊液26例、尿液10例、脓汁及干酪物10例、其他7例,对照组非结核病例20例,并以3D培养法为金标准,评价2种检测方法对结核性液体标本的诊断价值。结果 PhaB法、3D培养法检测阳性率分别为43.4%(53/122)、32.8%(40/122);PhaB法与3D培养法对比,差异有统计学意义(χ2=5.87,P<0.05);以3D培养法结果为金标准,则PhaB法检测准确度、敏感性为92.5%和100%。结论①PhaB法检测结核性液体标本具有较高的敏感性和特异性,且操作简便、快速,可以在肺外结核诊断中常规应用;②3D培养法可做分枝杆菌药物敏感性试验和菌型鉴定,但需时较长。Objective This research is to assess of the phage amplified biologically assay(PhaB) in the detection of the tuberculosis in humoral specimens.Methods The humoral samples of 142 patients were detected by PhaB method and BacT/ALERT 3D rapid culture method respectively,which included 122 tuberculosis(69 pleural effusion,26 CSF,10 urine,10 pus and cheese material,7 others and 20 non-tuberculosis.Results Tuberculosis positive rate detected by PhaB and BacT/ALERT 3D rapid culture method were 43.4%(53/122) and 32.8%(40/122),respectively.Taking 3D rapid culture method as the golden standard,the sensitivity and specificity of PhaB were 92.5% and 100%.Conclusion PhaB method is a simple and rapid method to detect the tuberculosis in humoral specimens with a relatively higher sensitivity and specificity than the conventional methods.Thus the PhaB method is accessible and applicable in the clinical use for the tuberculosis detection in humoral specimens.
