摘  要:目的:探讨早产儿甲状腺功能特点及临床意义。方法:本院2008年7月至2010年7月收治的120例早产儿纳入研究,胎龄28-37周,产重1.105-3.014 kg,同期40例正常足月新生儿作为对照。分别在生后第1天和第7天检测上述新生儿静脉血血清游离三碘甲状腺原氨酸(FT3)、游离甲状腺激素(FT4)以及总促甲状腺激素(TSH)。结果:新生儿第1天FT3、FT4以及TSH水平均高于第7天,而早产儿上述指标均显著低于足月新生儿,且胎龄越低、产重越轻的早产儿越明显(P<0.05)。结论:早产儿下丘脑-垂体-甲状腺轴发育不成熟,生后甲状腺功能可降低,有必要对早产儿生后进行甲状腺功能监测。Objective: To explore the characteristics of thyroid function and clinical significance in premature infant. Method: 120 cases of premature infants were admitted into the study with 28-37 weeks of gestational age and 1.105-3.014 kg birth weight. Forty full term neonates were reeruited as the control group. The values of serum free triodothyronine (FT3), free thyroid hormones (FT4) and thyroid stimulating hormone (TSH) were measured on birth and 7 days after birth respectively. Result: The values of FT3, FT4 and TSH on the first day were significantly higher than those in the seventh day for the all neonates(P〈0.05). The values of premature children were significantly lower than those of full-term infants, especially for neonates with lower gestational age and birth weight (P〈0.05). Conclusion:The hypothalamus-pituitary-thyroid axis on the premature infants is immature and the thyroid function is low. It is neeessary to detect thyroid function on the premature after delivered.
