辛伐他汀对自体移植静脉内膜增生的抑制作用

摘　　要：目的研究辛伐他汀对自体移植静脉内膜增生的影响。方法 48只健康成年家兔随机均分为辛伐他汀治疗组(A组)和生理盐水对照组(C组)。建立自体静脉旁路移植模型,术后3、7、28 d切取移植静脉,观察组织病理变化,测量新生内膜厚度及面积,检测静脉壁增殖细胞核抗原(PCNA)表达,RT-PCR检测静脉壁基质金属蛋白酶(MMP)2、MMP-9 mRNA表达,明胶酶谱法检测MMP-2、MMP-9活性。结果与C组比较,A组术后内膜增生、移植静脉壁阳性细胞数以及MMP-2、MMP-9 mRNA表达和活性均明显减少(P<0.05)。结论辛伐他汀可抑制自体移植静脉内膜增生,其作用机制可能是通过抑制血管平滑肌细胞(VSMC)增殖并限制其向内膜迁移而实现的。Objective To investigate the effect of simvastatin on intimal hyperplasia in autogenous vein grafts.Methods Forty-eight rabbits were randomly and equally divided into two groups of A（treated with simvastatin） and C（treated with normal saline as the control）.Autogenous vein grafts model was established and vein samples were taken on the 3rd,7th and 28th day after operation.Pathological changes of tissues were observed and intimal thickness and area were measured.The cell proliferation was analyzed by immunohistochemical detection of proliferating cell nuclear antigen（PCNA）.Expressions of matrix metalloproteinase（MMP）-2 and MMP-9 mRNA and activity in the vein grafts were detected by RT-PCR and substrate gelatin zymography,respectively.Results Compared with group C,the intimal hyperplasia after operation,positive cell numbers in autogenous vein grafts,expressions of MMP2 and MMP-9 mRNA and activity were significantly decreased in group A（P0.05）.Conclusion Simvastatin can inhibit intimal hyperplasia in autogenous vein grafts probably via combined inhibition of vascular smooth muscle cells（VSMC） proliferation and migration.
