逆行交锁髓内钉与锁定加压钢板治疗肱骨干骨折的疗效比较

摘　　要：目的研究逆行交锁髓内钉和锁定加压钢板治疗肱骨干骨折的临床疗效。方法选取29例肱骨干骨折患者,采用逆行交锁髓内钉固定15例,锁定钢板系统内固定14例,临床疗效评价包括手术时间、出血量、骨折愈合情况、桡神经麻痹、放射线骨折线消失时间、延迟愈合和发生率。结果平均随访1.5年,逆行交锁髓内钉在手术时间、出血量、临床骨骼愈合时间、桡神经麻痹、放射线骨折线消失时间及骨骼延迟愈合率等方面比较差异有统计学意义(P<0.05)。结论对于新鲜闭合性骨折,逆行交锁髓内钉在减少手术时间,手术出血量、缩短骨折愈合时间及减少并发症的发生等方面,均较锁定钢板更具优势,值得临床应用和推广。Objective To study the clinical efficacy of retrograde interlocking intramedullary nails versus locking compression plates in the treatment of adult humera1 shaft fracture. Methods 29 cases of humeral shaft fractures were selected, 15 cases of which treated with retrograde interlocking intramedullary nails, 14 cases treated with locking compression plates.Clinical effect evaluation included operation time, blood loss, fracture healing, radial nerve palsy, fracture line disappeared radiation time, and incidence of delayed union. Results The average followed-up of 1.5 years, retrograde intramedullary nail with locking compression plate in the operative time, blood loss, clinical bone healing time and radial nerve palsy, radiation and bone fracture line disappeared time delayed union rate and so the statistical value were statistically significant（P0.05）. Conclusion For fresh closed fractures, retrograde intramedullary nail in the reduction of operation time, blood loss, shorten the healing time and reduce complications in terms of advantages compared with locking plate, is worthy of clinical application and promotion.
