摘　　要：目的探讨短节段钉棒系统结合伤椎椎弓根钉固定治疗胸腰椎骨折的手术方法、意义。方法采用肌间隙入路短节段经伤椎椎弓根螺钉内固定,治疗椎体压缩性骨折和轻中度爆裂性骨折38例,了解术后内固定及疼痛情况变化。结果 38例患者均得到6～24个月随访。术后0周至末次随访时R值及Cobb角,与术前相比均明显降低(P<0.05)。结论采用肌间隙入路短节段钉棒系统结合伤椎椎弓根钉固定治疗胸腰椎骨折切实有效。Objective To discuss indications,operation method and clinical results of wiltse paraspinal approach short-segment pedicle screw fixation for thoracolumbar vertebral fracture.Methods 38 cases with thoracolumbar vertebral fracture underwent wiltse paraspinal approach short-segment pedicle screw fixation.Preoperative and postoperative VAS score,segmental lordosis（Cobb＇s angle）,R value,VSA score and internal fixation condition were assessed and compared clinically.Results All patients were followed up 6~24 months,which showed there was apparente statistical difference upon VAS score,segmental lordosis and R value between preoperative and postoperative（P0.05）.Conclusion Wiltse paraspinal approach short-segment pedicle screw fixation is an adequate and effective procedure for thoracolumbar vertebral fractures.
