清热解毒法治疗急性缺血性中风的系统综述

摘　　要：目的:对清热解毒法治疗急性缺血性中风的疗效进行系统评价。方法:系统检索Cochrane图书馆、PubMed、EMbase、CBM、CNKI、VIP和万方数据库,全面收集清热解毒法治疗急性缺血性中风的随机对照试验(RCT),并追查纳入文献后的参考文献。对文献进行质量评价,并采用RevMan5进行Meta分析。结果:按照入选标准,有39项随机对照临床试验纳入,均为低质量的试验。Meta分析结果显示,清热解毒治法指导下的中药治疗急性缺血性中风与对照组相比,有效率方面显示显著差异(OR 3.12,95%CI 2.57 to 3.79),并在改善神经功能评分(SMD-3.13,95%CI-4.23 to-2.03)、调节炎症因子(TNF-α、IL-6)等方面,有显著作用。结论:清热解毒治法指导下的中药治疗急性缺血性中风与对照组相比,疗效有显著优势,并在改善神经功能评分、调节炎症因子(TNF-α、IL-6)等方面,有显著作用。但由于本研究纳入的原始文献质量不高,有待于更多高质量研究的开展以进一步证实其疗效和安全性。Objective： To evaluate the efficacy of clearing heat and removing toxicity therapy（清热解毒法） on cerebral ischemic stroke. Method： systematically search Cochrane online library, PubMed, EMbase, CBM, CNK/, VIP and Wangfang database, collect randomized controlled trial （RCT） of treatment cases of clearing heat and removing toxicity therapy on cerebral ischemic stroke, Randomized controlled clinical tri- Ms were screened and reviewed by meta-analysis in RevMan version 5 software, Result： thirty-nine controlled clinical trials were reviewed by meta-analysis. The result demonstrates that the effectiveness rate of treatment based on clearing heat and removing toxicity therapy show significant difference compared with control group （ OR 3.12,95% CI 2.57 to 3.79）, also improve neurological function rating score （ SMD- 3. 13,95% CI-4.23to-2.03） and regulate inflammatory factors （TNF-a,IL-6） significantly. Conclusion： Treatment based on clearing heat and removing toxicity therapy show advantage on efficacy and other indicators such as neurological function rating score and inflammatory factor compared with control group. But due to quality of the original literature, more study of high quality and good design is expected to confirm the effectiveness and safety of clearing heat and removing toxicity therapy.
