摘 要:目的观察螺内酯(Spi)对醛固酮(Ald)诱导的大鼠肾小球系膜细胞(RMCs)表达单核细胞趋化蛋白-1(MCP-1)的影响,探讨其肾脏保护机制。方法体外培养RMCs,分组如下:NG组(葡萄糖浓度5.6 mmol/L),Ald组(Ald 10-7mol/L),A组(Spi 10-7 mol/L+Ald 10-7 mol/L),B组(Spi10-8 mol/L+Ald 10-7 mol/L),C组(Spi 10-9 mol/L+Ald10-7 mol/L)。体外培养48 h后收集各组细胞和上清液,以RT-PCR检测MCP-1、Ald受体(MR)和11β-羟类固醇脱氢酶2(11β-HSD2)的mRNA表达,ELISA法检测细胞上清中MCP-1蛋白浓度。结果 RMCs表达MCP-1、MR和11β-HSD2的mRNA;与NG组比较,Ald组RMCs MCP-1 mRNA和蛋白表达均明显增加;与Ald组比较,A、B组RMCs MCP-1mRNA和蛋白表达明显下降(P<0.01),C组RMCs上清液MCP-1蛋白水平明显降低(P<0.05),但MCP-1 mRNA表达无明显改变。结论 Spi可有效抑制Ald诱导的RMCs中MCP-1 mRNA的表达和蛋白合成,该作用可能与其肾脏保护部分有关。Objective To study the renoprotective mechanisms of spironolactone(Spi) by observing its effect on expression of monocyte chemoattractant protein-1(MCP-1) induced by aldosterone(Ald) in rat mesangial cells(RMCs).Methods RMCs were cultured in vitro and then divided into five group: normal group,group Ald(Ald 10^-7 mol/L),group A(Spi 10^-7 mol/L and Ald 10^-7 mol/L),group B(Spi 10-8 mol/L and Ald 10^-7 mol/L),group C(Spi 10^-9 mol/L and Ald 10^-7 mol/L).RT-PCR was used to measure the expression of mineralocorticoid receptor(MR),l1β-hydroxysteroid dehydrogenase(11β-HSD2) and MCP-1 mRNA.The levels of MCP-1 in the supernatants were determined by enzyme-linked immunosorbant assay(ELISA).Results The expressions of MR,11β-HSD2 and MCP-1 mRNA were detectable in RMCs by RT-PCR.After treatment of 10^-7 mol/L Ald for 48 h,the levels of MCP-1 mRNA and protein were significantly higher than that in the baseline levels(P〈0.01).Treatments of Spi(10^-7 mol/L and 10-8 mol/L) could markedly down-regulate the expressions of MCP-1 mRNA and protein which stimulated by Ald(P〈0.01).The level of MCP-1 protein in group C was less than that in group Ald(P〈0.05),but there was no statistical difference in the levels of MCP-1 mRNA expression between group Ald and normal group(P〉0.05).Conclusion Spi can act on specificly binding with MR on RMCs,and significantly down-regulate the MCP-1 mRNA and protein expressions induced by Ald in RMCs,which may provide an experimental evidence for further evaluating its renoprotective effect.
