CD62_P、CD63、D-二聚体与下肢深静脉血栓形成的关系

摘　　要：目的探讨下肢深静脉血栓形成(LDVT)患者血小板表面P-选择素(CD62P)、溶酶体颗粒糖蛋白(CD63)和血浆D-二聚体(D-D)在治疗前后的变化规律。方法选择确诊为LDVT 31例患者为治疗组,28例为对照组,运用流式细胞仪检测血浆CD62P、CD63的阳性表达率,用ELISA方法检测血浆D-D,比较溶栓、抗凝、抗血小板治疗前后各项指标的变化,分析其与双下肢周径差的相关性。结果治疗组CD62P、CD63、D-D均高于对照组,治疗3 d后CD62P、CD63开始下降,且CD63、D-D与治疗前比较差异有显著性;治疗组治疗7 d后CD62P、CD63均较治疗前明显降低,差异有显著性(P<0.01),D-D降低到治疗前水平;CD62P、CD63的变化趋势和双下肢周径差变化相符。结论 CD62P、CD63、D-D可作为LDVT的良好检查指标,对LDVT的诊断及疗效判断具有重要临床意义。Objective To discuss the variation of platelet surface P-selectin selecting（CD62P）,lysosomal granule glycoprotein（CD63） and plasma D-dimer（D-D） in patients with deep venous thrombosis in lower limbs（LDVT） between before and after treatment.Methods 31 patients with confirmed LDVT and 28 normal subjects were included.CD62P and CD63 expressed on platelets surface were measured by flow cytometry,and plasma D-D was measured by ELISA.The changes of these parameters were compared with thrombolytic therapy,anticoagulation and anti-platelet aggregation activity and treatment later which explored the relationship between them and the perimeter of both lower limbs.Results In the patients with LDVT,the levels of CD62P,CD63 and D-D were significantly higher than those in the control group.After therapy 3 days,the levels of CD62P and CD63 started low,there was difference compared with the levels of CD63 and D-D before therapy.After therapy 7 days,the levels of CD62P and CD63 were significantly lower than those before therapy（P〈0.01）.D-D decreased to the levels before treatment.The change of CD62P,CD63 levels was accorded with the drop of the perimeter difference between the suffered limb and the healthy one.Conclusion The change of D-D and CD62P and CD63 is a better measurement of LDVT,which may be useful for diagnosis and evaluating curative effect of LDVT.
