原发性闭角型青光眼小梁切除术后眼球相关生物学参数的变化

摘　　要：目的观察原发性闭角型青光眼行小梁切除术前后眼球生物学参数的变化。方法对2007年2月至2008年12月在我院诊断为原发性闭角型青光眼并行小梁切除术且有6个月完整随访记录的26例(31眼)患者分别于小梁切除术前、术后1周、1个月及6个月行角膜曲率计、眼部A超及前房角镜检查,并观察角膜曲率、中央前房深度、眼轴长度及房角Scheie分级等生物学参数的变化。结果小梁切除术前、术后1周、1个月和6个月,眼轴长度分别为(21.98±0.42)mm、(21.55±0.46)mm、(21.67±0.41)mm和(21.76±0.40)mm,术后眼轴与术前相比缩短,差异均有统计学意义(均为P<0.05);术前、术后角膜水平曲率比较,差异无统计学意义(F=0.030,P>0.05);术后角膜垂直曲率较术前增加,差异有统计学意义(F=6.886,P<0.05);整体房角结构未见明显变化,其中2眼出现房角重新开放,29眼房角结构无明显变化;小梁切除术前,术后1周、1个月和6个月的中央前房深度分别为(2.13±0.97)mm、(1.98±0.11)mm、(2.13±0.10)mm和(2.15±0.10)mm,术后1周与术前相比呈变浅趋势,差异有统计学意义(t=2.462,P<0.05);术后1个月、6个月与术前相比,差异均无统计学意义(均为P>0.05)。结论小梁切除术对原发性闭角型青光眼的眼轴长度、垂直角膜曲率和中央前房深度等眼球相关生物结构影响明显,对水平角膜曲率和房角形态影响不明显。Objective To investigate the biological parameters of eyeball in primary angle-closure glaucoma before and after trabeculectomy. Methods About 26 patients（31 eyes） at our department who were diagnosed as primary angle-closure glaucoma and had the trabeculectomy from February 2007 to December 2008,and all were completely followed up for 6 months.All patients were taken the test of keratometer,type-A ultrasonic test and gonioscopy before and at 1 week,1 month and 6 months after operation.The changes of corneal curvature,anterior chamber depth,length of ocular axis and chamber angle Scheie classification were observed. Results Before and at 1 week,1 month and 6 months after operation,the length of ocular axis were（21.98±0.42）mm,（21.55±0.46）mm,（21.67±0.41）mm and（21.76±0.40）mm,respectively.After trabeculectomy,the ocular axis was significantly shortened,and there were statistical differences（all P〈0.05）.While no difference was found in horizontal corneal curvature between before and after trabeculectomy（F=0.030,P〉0.05）.The vertical corneal curvature markedly increased after operation（F=6.886,P〈0.05）.The overall structure of chamber angle had no obvious change,about chamber angles of 2 eyes reopened,and 29 eyes without obvious change;Before and at 1 week,1 month and 6 months after operation,the depth of the central anterior chamber were（2.13±0.97）mm,（1.98±0.11）mm,（2.13±0.10）mm and（2.15±0.10）mm.The depth of central anterior chamber became shallow at 1 week after operation.There was no obvious change in horizontal corneal curvature and the structure of overall chamber angle. Conclusions Trabeculectomy obviously affect the ocular axis,the vertical corneal curvature and the depth of central anterior chamber,while has no effect on horizontal corneal curvature and strucutre of chamber angle in primary angle-closure glaucoma.
