摘 要:长期以来,城乡医疗保障制度呈现出嵌入二元经济社会结构的制度特征。在全民医保的背景下,城乡居民基于经济联结和社会资本的行为选择,需要城乡医疗保障制度筹资、管理、支付、服务和环境的统筹衔接。基于东莞、太仓、成都、西安四地调查数据的回归分析发现,统筹地区的经济发展水平是实现城乡医保整合的重要前提,不同群体医保受益的均衡是实现城乡医保整合的重要保障。因此,统筹城乡医疗保障的核心在于公平筹资和均等受益;统筹发展方向是分阶段、有步骤地化异趋同,最终实现构建城乡居民健康保障体系的目标;统筹发展的政策路径可从制度框架、筹资机制、统筹层次、经办资源、管理体制、转移接续等方面展开。Under the background of universal health care,the urban and rural health care systemshave shown the institutional characters of inserting in the dual economic social structures.Basedon the urban and rural residents'behavior choices of economical joint and social capital,the urbanand rural health care systems need to be integrated on fund raising,management,payment,serviceand environment.Based on regression analysis on the survey data of Dongguan,Taicang,Chengdu and Xi'an,it is found that the level of economic development is the important premise torealize the integration of urban and rural health care,and balanced benefit of different groups isthe important guarantee.Therefore,the cores of urban and rural health care integration are thejustice financing and balanced benefit: The direction of the integrating development is to realizethe convergence step-by-step,and achieve the aim of constructing the national health care system.The corresponding policies can be deployed as follows: institutional framework,financingmechanism,integrating gradation,administrative resources,management system,transferring andcontinuous procedures and so on.
