摘  要:目的:评估呼出气一氧化氮(FeNO)检测在咳嗽变异性哮喘(CVA)诊断中的价值。方法:2010年7月至2010年9月因慢性咳嗽就诊的患者共103例。对所有纳入研究的患者分别行FeNO检测和支气管舒张试验。依据病史及FeNO检查结果作出慢性咳嗽病因的初始诊断,在3个月的随访期后作出最终诊断。结果:完成随访的98例患者中的86例依据FeNO明确诊断,FeNO诊断CVA的特异度为90.6%,阳性预测值为88.1%,其灵敏度为84.4%,高于支气管舒张试验(57.8%)。结论:FeNO在CVA诊断中具有重要价值。低与高的FeNO值在CVA诊断中均具有临床意义。在CVA的诊断中,FeNO比支气管舒张试验的灵敏度更高。Objective To evaluate the diagnostic value of fraction exhaled nitric oxide(FeNO) test in patients with cough variant asthma(CVA).Methods Patients with chronic cough for at least eight weeks from July 2010 to September 2010 were enrolled.FeNO test and bronchodilator test were performed in all of the patients.Initial diagnosis was made by the value of FeNO and the clinical manifestations.Final diagnosis was made after 3-month follow-up.Results Of the 98 patients completed the follow up,86 patients had their diagnosis established by FeNO.The optimal cut off value was 35 ppb which was capable of differentiating CVA and non-CVA with a specificity of 90.6%,and positive predictive value of 88.1%.The sensitivity of FeNO test was higher than that of bronchodilator test in the diagnosis of CVA(84.4% vs 57.8%).Conclusions FeNO measurement is of great value in the diagnosis of CVA.Both low and high FeNO values have clinical significance.The sensitivity of FeNO test was higher than that of bronchodilator test in the diagnosis of CVA.
