摘 要:目的:利用64层螺旋CT三维最大密度投影法重建显示SPN与其肺内血管关系进行分析并分型,探讨其对判断结节良恶性的诊断价值。方法:37例SPN患者,行64层螺旋CT增强扫描后行3D MIP重建观察结节与血管关系并分型。结果:37例3D MIP重建图像中,35例与血管关系显示良好,其中恶性22例,良性15例,表现为Ⅰ型表现的11例,恶性SPN10例,良性SPN1例;Ⅱ型表现为8例,恶性4例,良性4例;Ⅲ型表现为6例,全部为恶性SPN,Ⅳ型表现为6例,恶性1例,良性5例;Ⅴ型表现为4例,全部为良性SPN;表现为Ⅰ型和Ⅲ型血管关系的倾向于恶性,表现为Ⅳ型和Ⅴ型血管关系的结节倾向于良性,良恶性差异有统计学意义(P<0.05)。结论:64层螺旋CT增强扫描3D MIP重建可以准确显示SPN与血管的关系及其分型,对SPN良恶性的评价有重要价值。Objective:To investigate the relationship of solitary pulmonary nodule(SPN)with vascula r as well as its type by 64-slice spiral CT three-dimensions maximum-intensit y projection(3D MIP)reconstruction,the value of diagnosis was also studied. Methods:37 patients having SPN underwent 3D MIP reconstruction of vessels after enhanced 64-slice spiral CT.The type of vessels and the relation to SPN were analyzed. Results:37 images of 3D MIP reconstruction of vessels were clear in 35 patients,maligna nt 22 and benign 15.The SPN-vessels relationship was identified as 5 types,11 cases of type Ⅰ,malignant 10 cases and benign 1 case,8 cases of type Ⅱinclu ding malignant 4 cases and benign 4 cases,6 cases of type Ⅲ,all for malignant SPN,6 cases of type Ⅳ,1 case of malignant and 5 cases with benign pathology,4 cases of type Ⅴ,all for benign SPN;Malignant nodules most commonly showed typeⅠand typeⅢ,benign nodules often displayed type Ⅳ and type Ⅴ,there wer e significant difference between malignant cases and benign cases(P0.05). Conclusion:64-slice spiral CT 3D MIP reconstruction after enhanced can clearly show the relationship of SPN with vascular appearance,which is important to dif ferentiate malignant nodules with benign ones.
