喉罩通气全麻在乳腺癌根治术中的运用

摘　　要：目的：在于评价喉罩通气应用于乳腺癌根治术患者静脉复合全身麻醉时的安全性、优越性、可行性.方法：选择乳腺癌根治术患者90例分为3组,A组行喉罩通气,B组行气管插管通气,C组行面罩加口咽通气道通气.观察对比3组患者诱导期和拔除通气道前后循环变化、术中呼吸管理难度、术中动脉血气分析PaCO2值及术后咽喉不适的发生率.结果：放置通气道后和拔除通气道前B组循环波动明显大于A组和C组.术中呼吸管理难度A组明显小于B、C组.术中动脉血PaCO2值C组明显高于A、B组.苏醒期躁动及拔除通气道时呛咳A组明显少于B、C组.术后咽喉不适发生率A组明显少于B组.结论：喉罩通气能够避免面罩通气时的通气不足及二氧化碳蓄积,减少气管插管和拔管时的循环波动和许多并发症,为行乳腺癌根治术患者静脉复合全麻时优越的通气工具,值得推广使用.Objecitve： Lies in appraises the throat to cover the ventilation to apply in breast cancer root governing skill patient vein compound general anesthesia time the security, the superiority, the feasibility. Methods： The choice breast cancer root governing skill patient 90 examples divide into 3 groups, A group of good throats cover the ventilation,B group dispels symptoms such as bloating the tube intubation ventilation,C group of good face guards add the mouth to swallow the air passage ventilation. After the observation contrasts around 3 group of patient induction periods and the wiping out air passage in the cyclic change, the technique breathes the management difficulty,the technique median artery courage vigor analyzes the PaCO2 value and the technique the pharynx and larynx ill formation rate. Results：After the laying aside air passage and the wiping out air passage first B group of circular wave is bigger than A group and C group obviously. In the technique breathes the management difficulty A group to be smaller than obviously B,C group. The technique median artery blood PaCO2 value C group is higher than A,B group obviously. Regains consciousness the time moves restlessly and when the wiping out air passage chokes coughs A group obviously to be short in B,C group. After the technique the pharynx and larynx ill formation rate A group is short obviously in B group. Conclusion： Is the throat covers the ventilation to be able to avoid the face guard ventilates when the ventilation insufficiency and the carbon dioxide stores up,reduces time the trachea intubation and the cupping circular wave and many complications,for good breast cancer root governing skill patient vein compound general anesthesia when the superior ventilation tool, is worth promoting the use.
