多层螺旋CT对小儿气管支气管异物中的诊断分析

摘　　要：目的：对多层螺旋CT的多种成像技术对于小儿气管及支气管内异物的显影效果以及其在此类痰痛中的诊断价值进行探讨。方法：选择我院在2009年3月至1010年3月期间收治的24例气管、支气管内存在异物的患儿，进行螺旋CT扫描检查，后期的处理技术包括曲面重建（CMPR）、多平面重建（MPR）、仿真CT内窥镜（CTVE）、最小密度投影（Minp）等，在明确了诊断以后即进行内窥镜摘取手术。结果：24例气管及支气管异物患儿中，有6例为气管内异物、13例为右侧支气管内异物、5例为左侧支气管内异物，内窥镜所得结果均与CT的诊断结果一致。结论：多层螺旋CT能够直观的显示处异物所在住置、多少、大小以及其与周围组织间的关系等，尤其是曲面重建和多平面重建，很大程度上提高了手术前临床初步诊断的准确性，且可以明确的指导临床治疗以制定方案。多层螺旋CT应该作为小儿气管及支气管内异物病例的首选检查项目，以进行临床确诊。Objective： As well as it carries on the discussion to the multi-layered screw CT many kinds of image formation technology regarding the young child trachea and the endobronchial foreign matter development effect in this kind of disease diagnosis value. Methods：Chose my courtyard in from March,2009 to March,1010 the period admitting 24 example trachea,the endobronchial existence foreign matter trouble, carried on the screw CT scanning inspection, the later period processing technology reconstructs （CMPR）, the multilevel including the curved surface reconstructs （MPR）, simulation CT endoscope （CTVE）, the smallest density projects （Minp） and so on,will be being clear about the diagnosis later namely to carry on the endoscope to select the surgery. Resuits：24 example tracheae and in bronchial tube foreign matter trouble, some 6 examples for trachea in foreign matter, 13 examples for right flank endobronchial foreign matter, 5 examples for left side endobronehial foreign matter, endoscope obtained result with C T diagnosis result consistent. Conclusion：Multi-layered screw CT can the direct-viewing demonstration place foreign matter in the position, how many, the size as well as it organizes with periphery the relations and so on, in particular the curved surface reconstruction and the multilevel reconstruction,enhanced in front of to a great extent the surgery the clinical preliminary diagnosis accuracy, also may be clear about the instruction clinical treatment formulates the plan. Multi-layered screw CT should take the young child trachea and the endobronchial foreign matter case of illness first choice inspection project,carries on the clinical diagnosis.
