摘  要:目的探讨青年A型行为与儿茶酚胺/核因子κB(NF-κB)/促炎因子通路的关系,为阐明A型行为与心血管疾病关系提供依据。方法以明尼苏达多相人格调查表2(MMPI-2)A型行为量表评分将900名大一新生(男476人;女424人)分为A型(≥11分,159人)、中间型(6~10分,539人),B型(≤5分,202人)3组;采用酶联免疫吸附法检测血清肾上腺素、去甲肾上腺素、白细胞介素(IL)6、IL-18水平。随机在A型、B型、中间型行为者中各抽取8例(男女各4例),抽取各例外周血20mL,分成5组,分别在5组中加入多巴酚丁胺0、20、30、40和50μmol/L孵育18h后测定各组血清中IL-6和IL-18浓度,然后提取单核细胞用免疫组化法测定NF-κB蛋白表达变化。结果血清儿茶酚胺浓度与A型行为人格特征呈正相关(r=0.375~0.662,P<0.01);A型组血清肾上腺素、去甲肾上腺素高于中间型组和B型组;A型行为人格特征与血清IL-6和IL-18浓度呈正相关(r=0.294~0.778,P<0.05或P<0.01)。多巴酚丁胺呈剂量依赖性诱导人单核细胞NF-κB/促炎因子通路激活,以40μmol/L多巴酚丁胺为最适宜剂量;40μmol/L多巴酚丁胺诱导激动后,A和B型行为者的单核细胞NF-κBp65及血清IL-6和IL-18均明显上调(P<0.01或P<0.05);A型行为者的单核细胞NF-κBp65及血清IL-6和IL-18均高于B型行为(P<0.01)。结论 A型行为者非应激与应激时借助血清中较高浓度的儿茶酚胺诱导NF-κB/促炎因子通路持续处于激活状态,这可能是青年A型行为者的一个病理生理学特征。Objective Nuclear factor-κB(NF-κB)and pro-inflammatory cytokines play an important role in initiation and progression of atherosclerosis and coronary disease,type A behavior pattern(TABP)is lifestyle risk factor.The aim of the study was to investigate the association between TABP and activity of/NF-κB/pro-inflammatory cytokines signaling in young persons,and to explore the possible role of catecholamine in inducing activation of NF-κB/pro-inflammatory cytokines.Methods According to the MMPI-2 type A scale scores,900 college freshmen(male 476,female 424)were divided into type A(≥11,n=159),type middle(6-10,n=539),type B(≤5 scores,n=202)3 groups.The concentrations of serum norepinephrine(NE),epinephrine(E),pro-inflammatory cytokines [interleukin(IL)6,IL-18] in all subjects were measured with ELISA.8 subjects(male 4,female 4)were randomly selected from type A,type B and type middle group,respectively.20 mL peripheral blood sample collected from each subject was divided into 5 groups.The 5 groups were then incubated with various concentrations of Dobutamine(0,20,30,40,50 μmol/L)for 18 hours.The concentrations of serum IL-6 and IL-18 were measured by ELISA and NF-κB expression by immunohistochemistry.Results The concentration of serum E,NE and IL-6,IL-18 in type A group was significantly higher than that in group middle and in type B group(P0.01).The significant association were found among E,NE and TABP total scores,TABP subscale scores and also IL-6,IL-18(r=0.294-0.778,P0.05 or P0.01).After Dobutamine inducing,NF-κB expression as well as IL-6 and IL-18 concentrations were upregulated in a dose-dependent manner(P0.05 or P0.01).Both NF-κB and IL-6,IL-18 in type A group were significantly higher than in type B group(P0.01).Conclusion NF-κB/pro-inflammatory cytokines signaling is always activated during stress or not in young persons with TABP,catecholamine play a mediator role in inducing activation of the signaling.
