摘 要:目的:观察益气化瘀方对腰神经根压迫损伤后L4神经根组织Bax和Bcl-2的表达影响。方法:雄性SD大鼠(SPF级)40只按随机数字表法分为假手术组、模型组、弥可保组和益气化瘀方组,每组各10只。采用自制胶块压迫于大鼠右侧L4神经根背根节处,假手术组仅切开皮肤和椎旁肌。造模后假手术组和模型组给予生理盐水灌胃;弥可保组采用肌肉注射弥可保注射液;益气化瘀方组给予益气化瘀方煎剂灌胃,于造模10d后取右侧受压神经根。采用免疫组织化学法检测背根节神经细胞Bax和Bcl-2的蛋白表达情况;实时荧光定量聚合酶链反应法检测神经根组织Bax和Bcl-2的基因表达变化。结果:腰神经根压迫后,模型组的Bax蛋白和基因表达明显增高,其中基因表达与假手术组比较差异有统计学意义(P<0.01);而Bcl-2蛋白和基因表达明显减少,其中基因表达与假手术组比较差异有统计学意义(P<0.01)。益气化瘀方可减少Bax的蛋白和基因表达,其中基因表达低于模型组(P<0.01)。另外益气化瘀方可增加Bcl-2的蛋白表达,但对Bcl-2基因表达无明显影响。结论:腰神经根压迫模型可导致受压神经根组织Bax的过表达增加和Bcl-2的表达抑制。益气化瘀方可抑制神经根组织Bax的过表达;可增加Bcl-2的蛋白表达,但对其基因表达无明显影响。Objective:To observe the effect of Yiqihuayu recipe on the expression of Bax and Bcl-2 after lumbar nerve root compression injury.Methods: Forty male SD rats were randomly divided into four groups: control group,model group,methycobal group and Yiqihuayu recipe group.Surgery were performed on rats in the model group,the methycobal group and the Yiqihuayu recipe group with a micro silica gel placed on the right L4 dorsal root ganglion,while the rats in the control group were only received skin and paravertebral muscle incision.Rats in the methycobal group and the Yiqihuayu recipe group were given methycobal by intramuscular injection and Yiqihuayu recipe by gavage respectively.Rats in the control group and the model group gaved saline by gavage as equal amount as the Yiqihuayu recipe group.The compressed nerve roots were harvested at 10 days after treatment.Bax and Bcl-2 protein and mRNA expression in each rat were detected with immunohistochemistry and real time-polymerase chain reaction(real time-PCR) respectively.Results: Gene and protein expression of Bax was significantly increased in model group.There was significant difference in Bax gene expression compared with that in the control group(P0.01).Gene and protein expression of Bcl-2 was significantly reduced in model group after nerve root compressed.There was significant difference in Bcl-2 gene expression compared with that in the control group(P〈0.01).The Yiqihuayu recipe could reduce the expression of Bax,and the gene expression of Bax in Yiqihuayu recipe group was significantly lower than that in the model group(P〈0.01).And the Yiqihuayu recipe could improve the protein expression of Bcl-2,but didn't affect its gene expression.Conclusion:Lumbar nerve root compression injury resulted in the overexpression of Bax and the expression inhibition of Bcl-2.The Yiqihuayu recipe can inhibit the gene and protein expression of Bax and improve the protein expression of Bcl-2 after nerve root injury,but doesn't affect the gene expre
