摘  要:目的探讨基因芯片检测系统在分枝杆菌属鉴定及耐药性分析的临床应用价值,以及不同类型标本检出率的差异。方法应用基因芯片技术,对患者的痰液、脓液、胸腹水、脑脊液等标本进行分枝杆菌属DNA检测,进一步对结核分枝杆菌阳性标本进行耐药性分析。结果 900份临床样本的鉴定结果总阳性率为10.6%,洗肉水、穿刺物、脓液等阳性率较高;进一步对40份结核阳性标本进行耐利福平基因rpoB及耐异烟肼kat Gi、nhA基因检测,共有9例检测到了突变的耐药基因。结论基因芯片检测系统可以直接进行分枝杆菌属鉴定及耐药性分析,适用于不同类型标本,简便快速,灵敏度高,特异性好。OBJECTIVE To evaluate the gene chip detection system for the mycobacterial species identification and drug resistance genes mutation,as well as the detection rate of different types of samples.METHODS The sputum,pus,pleural effusion,cerebrospinal fluid and other specimens from 900 cases were detected Mycobacterium DNA by gene chip.Mycobacterium tuberculosis positive samples were furtherly analyzed the drug-resistant genes.RESULTS Of clinical 900 cases,the positive rate was 10.6%,puncture material and pus had higher positive rate.In the 40 positive tuberculosis samples,there were detected resistance gene mutations in nine cases.CONCLUSION The gene chip system can directly fulfill Mycobacterium identification and drug resistance genes analysis,which is one of the simple,rapid,high sensitivity methods for different clinical types of samples.
