摘 要:目的了解呼吸科病区2007-2009年临床用药与铜绿假单胞菌(PAE)的临床分离率之间的宏观量化关系,为临床用药提供依据。方法统计呼吸科用药情况,将其换算成每100住院人天所消耗的限定日剂量(DDD/100人天),并监测PAE的分离率,最后对两者进行相关分析。结果假设不考虑病原菌检出时间滞后因素,三代头孢类的头孢曲松使用强度变化与PAE分离率变化之间的相关系数r=-0.697,P=0.012,n=12,差异有统计学意义,且两者呈负相关;假设考虑病原菌检出时间滞后因素,碳青霉烯类的亚胺培南/西司他丁使用强度变化与PAE分离率变化之间的相关系数r=0.675,P=0.023,n=11,差异有统计学意义,且两者呈正相关。结论用药强度与病原菌分离率的宏观量化结果,提示两者之间存在关系的趋势走向,结合体外药敏试验,有助于准确把握用药方向。OBJECTIVE To investigate the quantitive relationship between the pressure of medicative administration and the clinical isolation rate of Pseudomonas aeruginosa in respiratory wards.METHODS The data of consumption of different kinds of drugs were collected from Jan 2007 to Dec 2009 quartely,and transformed to the antibiotic pressure index expressed as the number of defined daily doses(DDD) per 100 patients per day,at the same time,clinical isolation rate of pathogenic P.aeruginosa was calculated.The correlation coefficient between pressure of medicative administration and the isolation rate of P.aeruginosa was calculated.RESULTS Ignoring the delayed effect of drug,the statistics significance correlation was found in ceftriaxone(r=-0.697,P=0.012,n=12)and considering of the delayed effect of drug,the statistics significance correlation was found in Imipenem(r=0.675,P=0.023,n=12).CONCLUSION The results suggest that the quantitive relationship between the pressure of medicative administration and the clinical isolation rate of bacteria can be used with bacterial susceptibility testing.for adjusting the comsumption of drugs.
