摘  要:目的了解中山地区临床常见革兰阴性菌耐药谱变迁,指导临床合理用药。方法应用回顾性调查方法对2007年1月-2008年12月中山市人民医院分离的常见革兰阴性菌耐药性进行统计分析。结果临床共分离出大肠埃希菌985株,肺炎克雷伯菌512株,铜绿假单胞菌1944株,鲍氏不动杆菌505株,对大肠埃希菌敏感率高的是亚胺培南,哌拉西林/他唑巴坦,敏感率分别为98.4%、95.4%;对肺炎克雷伯菌敏感率高的是亚胺培南、哌拉西林/他唑巴坦、阿莫西林/克拉维酸敏感率分别为98.6%、87.6%、81.4%;对铜绿假单胞菌敏感率高的是加替沙星较敏感,敏感率100.0%,头孢他啶、哌拉西林/他唑巴坦敏感率分别为83.7%、83.1%;对鲍氏不动杆菌敏感率高的是哌拉西林/他唑巴坦、亚胺培南,分别为100.0%、78.3%,4种细菌对其余药物中多数均有较高程度耐药。结论 2007年1月-2008年12月中山地区临床常见革兰阴性菌耐药性日趋严重,应加强细菌耐药性监测以指导临床进行合理规范的抗感染治疗。OBJECTIVE To analyze resistance spectrum of common clinical Gram-negative bacteria in Zhongshan area,and to instruct clinicians to administrate drugs reasonably.METHODS The drug resistance of common clinical Gram-negative bacteria isolated from Jan,2007 to Dec,2008 in the Zhongshan People′s Hospital were retrospectively analyzed.RESULTS A total of 985 kinds of Escherichia coli(ECO),512 kinds of Klebsiella pneumoniae(KPN),1944 kinds of Pseudomonas aeruginosa(PAE) and 505 kinds of Acinetobacter baumannii(ABA) were isolated from the clinic.The higher sensitive rate to ECO was imipenem(98.4%),piperacillin/tazobactam(95.4%);to KPN was imipenem(98.6%),piperacillin/tazobactam(87.6%),amoxicillin/clavulanic acid(81.4%);to PAE was gatifloxacin(100.0%),ceftazidime(83.7%),piperacillin/tazobactam(83.1%);and to ABA was piperacillin/tazobactam(100%),imipene(78.3%).CONCLUSION From Jan,2007 to Dec,2008,the drug resistance status of common clinical Gram-negative bacteria in Zhongshan district is more and more serious.And people should strengthen the monitoring of bacterial drug resistance so as to instruct the clinicians to have anti-infectious remedy reasonably and normatively.
