肿瘤医院ICU人工气道患者感染病原菌及耐药性分析

摘  要:目的探讨肿瘤医院ICU人工气道患者医院感染主要病原菌分布特点及耐药性,为临床治疗提供依据。方法对2年内ICU的62例人工气道合并医院感染患者各种标本分离的病原菌及耐药性进行回顾性分析。结果 62例患者共分离出病原菌354株,其中革兰阴性菌200株占56.50%,真菌110株占31.07%,革兰阳性菌44株占12.43%,主要病原菌的检出率白色假丝酵母菌居首位,占总菌株数29.10%;革兰阴性菌对亚胺培南、头孢哌酮/舒巴坦、哌拉西林/他唑巴坦耐药率低,而对第三代头孢耐药率较高,革兰阳性菌、真菌则分别对万古霉素、氟康唑保持较高的敏感性。结论肿瘤医院ICU人工气道患者感染病原菌以革兰阴性菌和真菌较常见,临床应重视病原菌的检查和耐药性监测。OBJECTIVE To explore the characteristics of pathogen distribution and their drug resistance in ICU patients with artificial-tract to provide information for clinical treatment.METHODS Within two years,62 patients admitted in our hospital ICU with artificial-tract and hospital-acquired infection were collected.The pathogens from all kinds of their specimen were isolated and the drug resistance was analyzed.RESULTS Totally 354 strains were isolated from 62 patients.Among them,200 were Gram-negative bacteria,accounting for 56.50%,110 were fungy,accounting for 31.07%,44 were Gram-positive bacteria,accounting for 12.43%.The most common pathogen was Candida albicans,accounting for 29.10%.The results displayed that Gram-negative bacteria were sensitive to inipenem,cefoperazone/sulbact,piperacillin/tazobac,and were resistant to 3rd cephalosporins.Gram-positive bacteria were sensitive to vancomycin and fungi were sencitive to fluconazole.CONCLUSION The most common pathogens in ICU patients with artificial-tract in our hospital are Gram-negative bacteria and fungi.Clinical doctors should attach importance to the inspection of pathogen and its drug resistance evaluation.
