摘  要:目的检测低碱性氨基酸培养基测定铜绿假单胞菌和鲍氏不动杆菌对帕尼培南体外敏感性的影响。方法收集2009年,来自3所教学医院的147株铜绿假单胞菌和150株鲍氏不动杆菌临床分离株;分别采用M-H琼脂培养基和1∶40 M-H琼脂培养基,以琼脂稀释法和纸片扩散法,测定菌株对帕尼培南、亚胺培南和美罗培南的体外敏感性,并使用M-H培养基采用琼脂稀释法测定其他7种临床常用药物的最低抑菌浓度(MIC)。结果铜绿假单胞菌对帕尼培南的药物敏感性,受培养基碱性氨基酸浓度影响,与人血清碱性氨基酸浓度相似的1∶40M-H琼脂培养基,能够增强碳青霉烯类尤其是帕尼培南的抗假单胞活性;帕尼培南对铜绿假单胞菌和鲍氏不动杆菌活性与亚胺培南和美罗培南相近,但具有更高的中介率。结论测定帕尼培南对铜绿假单胞菌的药物敏感性,应采用低碱性氨基酸培养基,而测定其对鲍氏不动杆菌的药物敏感性,应采用M-H琼脂培养基,以达到药物体外活性与临床有效性的一致。OBJECTIVE To test the impact of basic amino acid on in-vitro susceptibilities of panipenem against clinical isolates of Pseudomonas aeruginosa(PAE) and Acinetobacter baumannii(ABA).METHODS A total of 147 isolates of PAE and 150 isolates of ABA were collected from 3 teaching hospitals in China.Microbial susceptibilities of panipenem,imipenem and meropenem were determined by both agar dilution method and disk diffusion method using MH agar medium and 1∶40 MH agar medium.MICs of other 7 antimicrobial agents were also tested by agar dilution method recommended by CLSI.RESULTS Drug susceptibilities of PAE against carbapenems,especially panipenem,were higher in 1∶40 MHA,which has the similar concentration of basic amino acid with human serum.Panipenem showed similar activities to PAE and ABA with imipenem and meropenem,but with higher percentage of intermediate.CONCLUSION To get according result of in vitro susceptibilities to clinical effet of panipenem,antimicrobial susceptibilities against P.aeroginosa should be tested using low basic amino concentration medium,but for ABA dard MH agar medium should be used.
