针刺加穴位注射对面肌痉挛患者治疗前后外周血T细胞亚群的研究

摘　　要：目的:观察针刺加穴位注射对面肌痉挛患者治疗前后外周血T细胞亚群的影响并探讨其作用机理。方法:随机将68例面肌痉挛患者分为观察组、对照组各34例,同时设计正常组26例。观察组采用针刺加穴位注射,对照组采用卡马西平口服,疗程均为15 d,统计两组疗效。测定观察组、对照组患者治疗前、后和正常组外周血的T淋巴细胞亚群、B细胞、NK细胞水平。通过自身治疗前后对照及与正常组比较。结果:观察组比对照组两组总有效率比较P<0.001,两组比较差异有统计学意义;两组痊愈率比较P<0.001,两组比较差异有统计学意义。面肌痉挛患者均存在不同程度的T淋巴细胞亚群比值下调,B细胞、NK细胞偏高,经针灸治疗后T淋巴细胞亚群比值上调(P<0.05);B细胞、NK细胞有下降趋势(P>0.05),与药物治疗组治疗后相比无显著差异(P>0.05)。结论:针灸加穴位注射双向面肌痉挛效果满意,而且观察组没有副作用。针灸加穴位注射调节患者的T淋巴细胞、B细胞和NK细胞活性,其机制可能是通过对自身细胞免疫和体液免疫的调节而发挥作用。Objective：To observe the effect of combined therapy with acupuncture and injectio ad acumen on T cell subset in peripheral blood of patients with hemifacial spasm and to explore the mechanism.Methods：A total of 68 cases with hemifacial spasm were selected and randomly divided into observation group and control group by half.Another 26 normal persons were selected as normal control group.Patients in observation group were treated by combined therapy with acupuncture and injectio ad acumen,and oral carbamazepine was administered to patients in control group.The course was 15 d.And the level of T cell subset,B cell and NK cell were determined.Results：There were significant differences in total efficacy and cure rate（P0.001） between observation group and control group.The level of T cell subset of patients with hemifacial spasm decreased,and the levels of B cell and NK cell were increased.While T cell subset increased significantly（P0.05）,and the levels of B cell and NK cell decreased.There was no significant difference compared with normal control group（P0.05）.Conclusions：Combined therapy with acupuncture and injectio ad acumen shows satisfactory effect without side effect.It may work by regulating auto cell immunity and humoral immunity.
