彩色多普勒超声对下肢深静脉瓣功能不全的新探讨

摘　　要：目的探讨彩色多普勒超声(CDFI)探测下肢静脉瓣功能不全的新方法及临床应用价值。方法选择20例正常对照组及40例下肢深静脉瓣功能不全患者(共59条腿),应用CDFI技术在限定的条件下观察反流束的各项指标及血流动力学改变。结果单条彩色反流束宽度以及多条彩色反流束宽度占管腔宽度<30%者可临床观察,>50%有手术意义;彩色反流束长度占两瓣之间管腔长度的20%～40%可临床观察,>40%有手术意义。结论彩色多普勒是诊断下肢深静脉瓣功能不全的一种简便、可靠无创伤的方法,以往是通过测量反流频谱持续时间(VCT)来判断深静脉瓣功能的程度,本研究主要是在此基础上观察彩色反流束的宽度及长度占管腔的比率来判断下肢深静脉瓣功能,并为临床治疗提供依据,且本方法直观、方便,值得推广。Objective To check on color doppler flow imaging in diagnosing venous valvular insufficiency in lower extremity. Methods To chose the 20 healthy and 40 patients （total 59 legs） with venous valvular insufficiency and to apply a CDFI technique to carry on an observation under the condition. The change of anti-to flow each index sign and blood dynamics of bunch is discovered. Results If the width of a bunch of color anti-flow or several banches is 〈30% of chamber widthof the patient,the patient should be clinical observed,if 〉50%,if the length of a bunch of color anti-flow is 20% ＋40% of chamber length between the two valvular of the patient,the patient should be clinical observed,if ~40% operating the patient is meanful. Conclusion Color Doppler flow imaging is a simple, credibility method without wound to diagnose venous valvular insufficiency in lower extremity.ln former to judge venous valvular in lower extremity by measuring VCT, this research is to obersever the ratio of the color anti-flowed width and length of bunch and the chamber on this foundation to judge venous valvular in lower extremity, as provides the basis for the clinical treatment, this method is directly perceived,convenience,and worth expanding.
