摘 要:目的:探讨精索静脉曲张(VC)对青春期大鼠睾丸组织中睾酮浓度(ITC)、血清睾酮(T)、卵泡刺激素(FSH)、黄体生成素(LH)及其睾丸雄激素受体(AR)、卵泡刺激素受体(FSHR)和黄体生成素(LHR)的影响。方法:通过部分结扎左肾静脉方法制作左侧精索静脉曲张模型(EV),雄性Wistar大鼠36只,随机分为6组,EV持续2、4、8周组和相应接受假手术的对照组,每组各6只。于造模成功后2、4、8周取实验组及对照组下腔静脉血及左侧睾丸,放射免疫法检测ITC及血清T、FSH、LH浓度,并用免疫组化方法检测其受体AR、FSHR、LHR在睾丸组织中的表达,用TUNEL法检测细胞凋亡并计算生精细胞的凋亡指数(AI)。结果:VC后2周时,各激素平均灰度值未见明显变化,实验组LHR(138.77±7.41)明显上升(P﹤0.05);4周时实验组ITC(7.11±1.74)nmol/L较对照组(9.51±1.50)nmol/L及实验组2周组(9.01±0.98)nmol/L下降(P﹤0.05),8周时实验组血清T(0.38±0.11)nmol/L及ITC(4.46±1.24)nmol/L较实验组2周组[T(1.01±0.26)nmol/L、ITC(9.01±0.98)nmol/L]、4周组[T(0.93±0.17)nmol/L,ITC(7.11±1.74)nmol/L]均有明显差异(P﹤0.05)。实验组AI[(8.74±1.79)%、(19.08±2.31)%、(30.61±1.77)%]均明显高于对照组[(5.97±0.90)%、(5.35±1.05)%、(6.28±0.89%)],各EV组生精小管的超微结构损伤也随着精索静脉曲张持续时间的增加而逐渐严重。结论:VC可降低EV大鼠ITC、血清T浓度以及其受体AR的表达,使生精细胞AI增加并影响血清FSH、LH浓度及其睾丸受体的表达。Objective: To study the effect of experimental varicocele on intratesticular testosterone concentration(ITC),serum testosterone(T),follicle stimulating hormone(FSH),luteinizing hormone(LH) and androgen acceptor(AR),follicle stimulating hormone acceptor(FSHR),luteinizing hormone acceptor(LHR) in testis of adolescent rats.Methods: Establishing experimental left varicocele animal model by partial ligation of renal vein in male Wistar rats.The control rats subjected to sham operation.2,4 and 8 weeks after surgery,inferior vena cava blood and the testes of the EV models and controls were extirpated.The gonadal hormone concentration and ITC were studied using Radioimmunoassay and the AR,FSHR,LHR were studied using immunohistochemistry.The apoptosis index was detected by TUNEL.Results: 2 weeks after surgery,there was no significant change in the average gray value of ITC,scrum T,FSH,LH,but the expression of LHR(138.77±7.41) increased remarkable(P0.05).4 weeks after surgery,there was a remarkable decline of ITC(7.11±1.74) nmol/L compared with the control group(9.51±1.50) nmol/L and 2-week group(9.01±0.98) nmol/L(P0.05).8 weeks after surgery,there was a remarkable decline of scrum T(0.38±0.11) nmol/L and ITC(4.46±1.24) nmol/L compared with the 2-week group [T(1.01±0.26) nmol/L,ITC(9.01±0.98) nmol/L] and the 4-week group [T(0.93±0.17) nmol/L,ITC(7.11±1.74) nmol/L](P0.05).There were remarkable increasing of AI compared between the EV groups [(8.74±1.79)%,(19.08±2.31)%,(30.61±1.77)%] and the control groups [(5.97±0.90)%,(5.35±1.05)%,(6.28±0.89)%].The damage to the ultrastructure of seminiferous tubules was aggravated with the prolonging of varicocele.Conclusion: Experimental varicocele decreases ITC,serum T and the expression of AR,increases the AI of germ cells,and affect the concentration of serum FSH,LH and their acceptor.
